HL7 FHIR: Ontological Reinterpretation of Medication Resources.

"A solid ontology-based analysis with a rigorous formal mapping for correctness" is one of the ten reasons why the HL7 standard Fast Healthcare Interoperability Resources (FHIR) is advertised to be better than other standards for EHR interoperability. In this paper, we aim at contributing to this formal analysis by proposing an RDF representation of a subset of FHIR resources based on a highly constrained top-level ontology and guided by the use of a set of Content Ontology Design Patterns (Content ODPs) for representing clinical information. We exemplify this by reinterpreting FHIR medication resources. Although a manual task now, we foresee a possible automatic translation by using RDF shapes.

This study presents a web application for predicting cardiovascular disease (CVD) and hypertension (HTN) among mine workers using machine learning (ML) techniques. The dataset, collected from 699 participants at the Gol-Gohar mine in Iran between 2016 and 2020, includes demographic, occupational, lifestyle, and medical information. After preprocessing and feature engineering, the Random Forest algorithm was identified as the best-performing model, achieving 99% accuracy for HTN prediction and 97% for CVD, outperforming other algorithms such as Logistic Regression and Support Vector Machines.

The general control non-repressible 5 (GCN5)-related N-acetyltransferase (GNAT) SbzI, in the biosynthesis of the sulfonamide antibiotic altemicidin, catalyzes the transfer of the 2-sulfamoylacetyl (2-SA) moiety onto 6-azatetrahydroindane dinucleotide. While most GNAT superfamily utilize acyl-coenzyme A (acyl-CoA) as substrates, SbzI recognizes a carrier-protein (CP)-tethered 2-SA substrate. Moreover, SbzI is the only naturally occurring enzyme that catalyzes the direct incorporation of sulfonamide, a valuable pharmacophore in medicinal chemistry.

Background: Subcutaneous emicizumab, a factor VIII (FVIII)-mimicking bispecific monoclonal antibody, can effectively prevent bleeds in haemophilia A (HA) patients with/without inhibitors; however, its standard-dose regimens are financially burdensome. Low-dose emicizumab prophylaxis may alternatively be applied to noninhibitor HA patients in resource-limited settings.

Methods: During 2023, Thai patients with noninhibitor severe HA or moderate HA with severe bleeding phenotype (historical annualized bleeding rate [ABR] >5 bleeds/year before regular FVIII prophylaxis) who received low-/intermediate-dose FVIII secondary prophylaxis ≥8 months were enrolled.
