Purpose: To highlight the role of constructive interference steady state (CISS) magnetic resonance imaging (MRI) in the diagnosis of Tolosa-Hunt Syndrome (THS).
Case Report: We describe a case of THS in a 55-year-old woman presenting with left painful opthalmoplegia that was diagnosed by CISS MRI. Patient responded to steroid treatment and the lesion resolved.
Conclusion: Imaging with MRI can help in making the diagnosis of THS by demonstrating an enhancing soft tissue lesion in the cavernous sinus and orbital apex resolving with steroids. CISS MRI is a sensitive sequence for diagnosis and follow-up imaging in THS.

Tolosa Hunt syndrome (THS) is characterized by painful ophthalmoplegia preceded by retro orbital headache. First described in year 1952, it is a diagnosis of exclusion and is extremely rare entity Here we present a case report of syndrome in a female patient who presented with 03 weeks of progressive unilateral retro orbital headache followed by acute onset ptosis and complete ophthalmoplegia in right eye. On evaluation (brain imaging) and after ruling out all the other differentials, she was diagnosed as a case of THS.

Invasive aspergillosis is a rare but severe fungal infection primarily affecting immunocompromised individuals. The Disease-2019 (COVID-19) pandemic has introduced new complexities in managing aspergillosis due to the widespread use of corticosteroids for treating COVID-19-related respiratory distress, which can increase susceptibility to fungal infections. Introduction: Tolosa-Hunt syndrome (THS) is a disorder related to inflammation of cavernous sinus and superior orbital fissure that usually presents with ophthalmoplegia and oculomotor nerve palsies. The etiology of the syndrome is unknown and the diagnosis is set by exclusion of other clinical conditions that manifest in a similar way. Intracranial aneurysms, such as intracavernous ones, should be included in the differential diagnosis as they can compress cranial nerves leading to similar clinical presentation.
