Background And Purpose: Several clinical measures and biomarkers are associated with motor recovery after stroke, but none are used to guide rehabilitation for individual patients. The objective of this study was to evaluate the implementation of upper limb predictions in stroke rehabilitation, by combining clinical measures and biomarkers using the Predict Recovery Potential (PREP) algorithm.
Methods: Predictions were provided for patients in the implementation group (n=110) and withheld from the comparison group (n=82). Predictions guided rehabilitation therapy focus for patients in the implementation group. The effects of predictive information on clinical practice (length of stay, therapist confidence, therapy content, and dose) were evaluated. Clinical outcomes (upper limb function, impairment and use, independence, and quality of life) were measured 3 and 6 months poststroke. The primary clinical practice outcome was inpatient length of stay. The primary clinical outcome was Action Research Arm Test score 3 months poststroke.
Results: Length of stay was 1 week shorter for the implementation group (11 days; 95% confidence interval, 9-13 days) than the comparison group (17 days; 95% confidence interval, 14-21 days; =0.001), controlling for upper limb impairment, age, sex, and comorbidities. Therapists were more confident (=0.004) and modified therapy content according to predictions for the implementation group (<0.05). The algorithm correctly predicted the primary clinical outcome for 80% of patients in both groups. There were no adverse effects of algorithm implementation on patient outcomes at 3 or 6 months poststroke.
Conclusions: PREP algorithm predictions modify therapy content and increase rehabilitation efficiency after stroke without compromising clinical outcome.

Objective: The aim of this study was to assess the effectiveness of a postoperative multimodal pain control protocol on perioperative pain scores in children undergoing decompression for Chiari type I malformation (CM-I).
Methods: This retrospective matched cohort study included patients < 21 years of age who underwent elective suboccipital craniectomy and C1 laminectomy for CM-I with or without duraplasty at a single center from January 2020 to July 2023. A standardized, multimodal postoperative pain protocol was implemented in August 2021 that did not use narcotic patient-controlled analgesia.

Recently, the rapid increase in global plastics production has caused various ecological and economic issues, worsened by poor material and waste management. Among the market-based instruments that could help mitigate the environmental impacts of plastics throughout their life-cycle, we evaluate the advantages and limitations of incorporating a cap-and-trade (CAT) system into future policy mixes. Our aim is to inspire further investigation of CAT's feasibility rather than presenting it as the ultimate solution.

Purpose: We aimed to determine whether implementation of clinical decision support (CDS) tool integrated into the electronic health record (EHR) of a multi-site academic medical center increased the proportion of patients with American Urological Association (AUA) "high risk" microscopic hematuria (MH) who receive guideline concordant evaluations.
Materials And Methods: We conducted a two-arm cluster randomized quality improvement project in which 202 ambulatory sites from a large health system were randomized to either have their physicians receive at time of test results an automated CDS alert for patients with 'high-risk' MH with associated recommendations for imaging and cystoscopy (intervention) or usual care (control). Primary outcome was met if a patient underwent both imaging and cystoscopy within 180 days from MH result.

Introduction: Uptake of voluntary medical male circumcision (VMMC) remains a challenge in many settings. Innovative implementation strategies are required to scale-up VMMC uptake.
Methodology: RITe was a multi-faceted intervention comprising transport reimbursement (R), intensified health education (IHE) and SMS/Telephone tracing (Te), which increased the uptake of VMMC among uncircumcised men with sexually transmitted infections (STIs) in Malawi.
Objective: To clarify the screening behavior and influencing factors of females with breast cancer and cervical cancer in suburban areas and to provide a scientific basis for the subsequent implementation of targeted health education, intervention measures and the formulation of relevant policies.
Methods: This study used a multi-stage stratified random sampling method to select 4, 000 women in urban and rural areas of Beijing to analyze their behavior, basic situation, and influencing factors regarding cervical and breast cancer screening.
Results: The sample size of the final included valid analysis was 3861 people, and the screening rate was 27.
