Elderly onset Rheumatoid arthritis (EORA) has important clinical distinctions when compared with younger onset RA (YORA). In knee arthritis of elderly patients, infection, crystal-induced arthritis or EORA should be suspected if elevation of CRP in the preoperative examination and turbid joint effusion in their knee joint are found. Furthermore, if joint swelling and effusion remain after performing total knee arthroplasty (TKA), the infection after TKA, implant debris-related arthritis and EORA should be considered. However, it is difficult to diagnose patients as EORA if Rheumatoid factor (RF) and anti-cyclic citrullinated peptide antibody (ACPA) are negative. The differential diagnosis is very important.

Purpose: This study aims to compare the prevalence of depression and related geriatric syndromes in earlier-onset rheumatoid arthritis (EORA) patients, who have experienced prolonged inflammation and medication use, with those with late-onset rheumatoid arthritis (LORA) patients, who often present with an acute and severe course.
Methods: In this multidisciplinary study, patients with EORA and LORA aged 60 and over who were referred to a tertiary rheumatology clinic underwent a geronto-rheumatologic evaluation. Muscle mass and handgrip strength, cognitive function, nutritional status, Fried frailty index, fall history, gait speed, depression according to Geriatric Depression Scale and Insomnia Severity Index were recorded.

Rheumatoid arthritis (RA) is a chronic inflammatory disease with a prevalence of 1%, mainly affecting women aged 25-45. It is classified by the age of onset into young-onset rheumatoid arthritis (YORA, 16-65 years) and elderly-onset rheumatoid arthritis (EORA, over 65 years), with EORA often presenting suddenly with systemic symptoms and large joint involvement due to age-related immune changes.
