Osteosarcoma (OS) is a common primary malignant tumor of bones that produces osteoid matrix. Telangiectatic osteosarcoma (TOS) is a rare variant of OS. It affects the long bones especially the lower end of femur and the upper ends of tibia and humerus, a distribution similar to the conventional osteosarcoma. The rib involvement is very infrequent. We present a case of TOS of the rib that posed a diagnostic difficulty owing to its unusual location and to its resemblance to giant cell tumor and aneurysmal bone cyst. Correspondence.

Introduction & Importance: Parosteal osteosarcoma is an uncommon, low-grade, well differentiated malignant bone neoplasm with a more favorable prognosis compared to other types of osteosarcoma. Dedifferentiation is a well-known phenomenon, observed in 16-24.6 % of cases, leads to a combination of low-grade fibroblastic osteosarcoma and high-grade sarcoma.

Background: Telangiectatic osteosarcoma is rare and it rarely affects flat bones, especially the bones of the pelvis. It is uncommon for telangiectatic osteosarcoma to be considered as a differential diagnosis when assessing a large intrabdominal mass.
Case Report: We present our case of a 33-year-old African female who presented with a sizeable telangiectatic osteosarcoma of the left iliac bone.
