Granulomatous lesions, such as foreign body granuloma, idiopathic granulomatous mastitis (IGM), and sarcoidosis can mimic breast carcinoma. IGM is associated with elevated prolactin (eg, pregnancy or oral contraceptive use) and is usually subareolar. Infection, however, is also commonly subareolar.

They are primarily a psychiatric issue rather than a surgical condition and are associated with trichotillomania, with subsequent trichophagia, pica, and other psychiatric conditions usually seen in adolescent females. This case report describes an extremely rare diagnosis of Rapunzel syndrome in a 4-year-old female.

When it occurs as an isolated lesion in the breast, it presents unique diagnostic and therapeutic challenges due to its rarity and clinical similarity to other breast conditions.
Case Presentation: We report a case of a 60-year-old female presenting with a gradually enlarging mass in her right breast over six months.
