Objective: This study was designed to evaluate the effects calcium fructoborate (CFB) and sodium tetraborate (NTB) as supplements in Iraqi patients with active rheumatoid arthritis (RA) maintained on etanercept.

Materials And Methods: A double-blind randomized placebo-controlled clinical trial with 60 days treatment period was carried out at Baghdad Teaching Hospital, Medical city, Baghdad, Iraq. Eighty RA patients were randomized into three groups to receive either 220 mg/day CFB, 55 mg/day NTB in capsule dosage form (equivalent to 6 mg elemental Boron), or placebo formula once daily. Only 72 patients completed the study. All patients were clinically evaluated utilizing DAS28-erythrocyte sedimentation rate (ESR), simple disease activity index-C-reactive protein (CRP), and clinical disease activity index scores at baseline, and at the end of the study. Venous blood was obtained at baseline and after 60 days, and utilized for the measurement of ESR, hemoglobin, in addition to evaluation of high-sensitivity CRP (hsCRP), tumor necrosis factor-α (TNF-α), interleukin-1α (IL-1α) and IL-6.

Results: After 60 days, both types of boron significantly improve the clinical scores, in association with significant decrease in the serum levels of ESR, hsCRP, IL-1α, IL-6, and TNF-α with remarkable superiority for calcium fructoborate (CFB) over sodium tetraborate (NTB), compared to baseline and placebo-treated group.

Conclusion: The use of boron, as adjuvant with etanercept, has potentiated therapeutic outcomes in RA patients, and may be a new strategy to improve treatment, and avoid the problems associated with biologics utilized in RA treatment.

The mechanism of CFB in treating knee osteoarthritis is not yet clear and deserves further discussion. The C28/I2 cell was stimulated by TNF-α and the MIA-induced OA rat model were constructed, and then treated with a certain concentration of CFB. The effects of CFB on chondrocyte apoptosis, inflammatory response, and collagen matrix degradation were assessed.

Obesity is a global public health problem that can lead to mortality and morbidity. Studies on the pathophysiology of obesity for effective and safe treatments are focused on the mechanisms of adipogenesis. The association between boron treatment and weight loss has been reported, but its anti-adipogenic mechanisms and effects on preadipocytes remain unclear.

Article Synopsis
  • The study explores how boric acid and calcium fructoborate impact immune system cells, specifically Treg and Th17 cell populations, in patients with rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE).
  • It involved blood samples from newly diagnosed RA and SLE patients and healthy individuals, exposing isolated mononuclear cells to varying doses of boric acid and calcium fructoborate to analyze effects on cell populations and cytokine levels.
  • Results indicated that while high-dose boric acid improved Treg/Th17 ratios in healthy controls, it did not have the same effect in RA and SLE patients; however, both boron compounds showed a potential positive impact on inflammation markers in RA.

Carbon tetrachloride (CCl) is a xenbiotic that can cause cellular damage with free radical production. Calcium fructoborate (CFB) is a boron-based nutritional supplement with antioxidant properties. Calcium fructoborate used in our study is marketed by Future Ceutical Corporation as FruiteX-B, which has a chemical structure similar to the natural form of boron found in edible plants.

Sugar-borate esters have recently been reported to have anti-cancer potential. Among the sugar-borate esters, calcium fructoborate (CaFB) possesses beneficial effects on human health. Despite the beneficial effects of CaFB, there is a lack of knowledge about their mode of action in cancer.
