Physicochemical and Biological Characterization of Ti6Al4V Particles Obtained by Implantoplasty: An In Vitro Study. Part I.

Implantoplasty is a mechanical decontamination technique that consists of polishing the supra-osseous component of the dental implant with peri-implantitis. This technique releases metal particles in the form of metal swarf and dust into the peri-implant environment. In the present in vitro study, the following physicochemical characterization tests were carried out: specific surface area, granulometry, contact angle, crystalline structure, morphology, and ion release.

425 patients with mandibular angle fractures were treated at the Hospital of Kaunas University of Medicine (HKUM) Clinic of Maxillo-Facial Surgery. Treatment included the application of closed fracture fragments fixation methods (wire splint fixation, and Kirschner wire osteosynthesis), and methods of open fixation--osteosynthesis using the supra-periosteal miniplate, and osteosynthesis using supra-osseous Zes Pol plate (the latter method was modified by the authors). Relative computerized densitometry showed that closed fixation methods result in a faster healing of fractures.

Osteosynthesis of mandibular fragments by the traditional and biomechanics-based methods was studied in dogs. The morphological picture was virtually the same in the immediate periods (5-10 days), whereas in remote periods (1, 3, and 6 months) osteogenesis was more adequate in the group where biomechanically based methods were used, particularly so with the supraosseous miniplates.

Having used since 1968 in MONICI the massive metal plates of different configuration and size, developed by Swiss association of osteosynthesis AO and Czechoslovakian company Poldy, the authors summarize the experience of great clinical material. Altogether, there have been operated over 1000 patients with recent and inveterate fractures of tubular bones, pseudoarthroses, incorrectly consolidated fractured fragments, and 603 of them in MONICI. Some of the implants have been adapted by the authors.

The report is based on the analysis of the treatment of 32 patients with false joints of the humerus after gunshot injuries. All of them were operated and had supraosseous osteosynthesis with an AO plate (6 cases) and osteosynthesis after Ilizarov (26 cases). When the false joint of the shoulder was associated with the radial nerve paralysis, the tendons were transplanted after Chaklin in 9 cases.
