AI Article Synopsis

  • Von Hippel-Lindau (VHL) disease is a rare genetic condition that can cause tumors and cysts in different body parts like the kidneys and pancreas.
  • A 38-year-old woman had VHL disease for ten years, and it affected both her pancreas and biliary system, which is a first report of this.
  • Her doctors used scans and genetic tests to diagnose her, and she chose to receive careful monitoring instead of aggressive treatment, which revealed some cysts but no harmful growths.

Article Abstract

Rationale: Von Hippel-Lindau (VHL) disease is a rare inherited, autosomal-dominant syndrome caused by heterozygous germline mutations in the VHL gene. VHL patients are prone to develop benign and malignant tumors and cysts in multiple organ systems involving kidneys, pancreas and central nervous system (CNS). The varied and complex clinical manifestations and radiological findings of VHL are of interest.

Patient Concerns: We report a 38-year-old woman with a ten-year history of VHL disease involving both pancreas and biliary system. To the best of our knowledge, direct involvement of the biliary system in VHL disease has never been reported.

Diagnoses: The diagnosis was established via computed tomography scan and was confirmed by genetic testing.

Interventions: The patient chose to receive conservative treatment and was followed up by magnetic resonance cholangiopancreatography and magnetic resonance imaging examination.

Outcomes: Renal angiomas and cysts were found during follow-up and there were no evidence of malignant change of the pancreas and biliary system.

Lessons: We described the first case of VHL-associated choledochal cysts and may present new visceral manifestations of VHL disease. Gastroenterologists should be aware of the clinical presentations of this rare disease for early detection of its life-threatening manifestations.

Background: Major mutations (e.g., KRAS, GNAS, TP53, SMAD4) in pancreatic cyst fluid (PCF) are useful for classifying and risk stratifying certain cyst types, particularly in cases with nondiagnostic cytology.

Proteolysis-Targeting Chimeras (PROTAC) are a bifunctional molecule that binds to a protein of interest (POI) and a ubiquitin ligase, thereby inducing the ubiquitination and degradation of POI. Many PROTACs currently utilize a limited number of ubiquitin ligases, such as von Hippel-Lindau (VHL) and Cereblon. Because these ubiquitin ligases are widely expressed in normal tissues, unexpected side effects can occur.

Background: Obesity is a risk factor for cardiovascular diseases and associated with reduced life expectancy metabolic bariatric surgery (MBS) is the treatment indicated when patients are unable to lose weight through lifestyle changes and medication alone. However, more evidence is necessary to show non-inferiority of e-health compared to in-person monitoring with regard to important parameters for the success of surgical treatment of obesity such as anthropometric changes.

Methods And Analyses: This review study will include cohort studies involving individuals with obesity and e-health or in-person patient monitoring before and after MBS.

Central nervous system hemangioblastoma (CNS-HB) is the most common manifestation of von Hippel-Lindau disease (VHL). The main axis of the CNS-HB pathway is the VHL-HIF signaling pathway. Recently, we proposed an alternative VHL-JAK-STAT pathway in CNS-HB.

Long-term surgical outcomes and prognosis of cervical spinal hemangioblastomas.

Clin Neurol Neurosurg

Department of Neurosurgery, Beijing Tiantan Hospital, Capital Medical University, Fengtai District, Beijing 100070, China.

Purpose: Hemangioblastomas (HBs) occurring in the cervical spinal region are infrequently reported. Surgical resection of cervical HBs poses a significant challenge, and the long-term therapeutic outcomes remain unclear.

Methods: A comprehensive retrospective analysis was conducted to review the treatment outcomes of patients with cervical HBs from 2011 to 2021.
