Introduction: Early recognition and treatment of stroke is paramount for good outcome. Transport distance may result in delayed arrival for revascularization therapy. We investigated how transport time and distance to the revascularization unit affected the probability of receiving intravenous thrombolysis in Denmark between 2015 and 2020, for patients calling the Emergency Medical Services within three hours of symptom onset.

We discuss the case of a 37-year-old male with ARVC who presented with acute onset of dizziness, imbalance, and vomiting, leading to the diagnosis of a top-of-basilar artery occlusion which was successfully treated. Case presentation This case highlights the diagnostic and acute treatment challenges in basilar artery occlusion (BAO) due to its non-specific symptoms and emphasizes the critical role of CT angiography in detecting occlusive thrombi for timely intervention.

Background: Approximately half of the patients with acute ischemic stroke who receive intravenous thrombolysis (IVT) do not achieve an excellent outcome. Remote ischemic conditioning (RIC) as a promising neuroprotective treatment may improve clinical outcomes in this population.
