The diagnosis and treatment of cancer of the major duodenal papilla in 83 patients are discussed. Radical operations were carried out in 71 (86%) patients; 60 were economical pancreatoduodenal resection (EPDR) and 11 were extended pancreatoduodenal ones (ExPDR). Postoperative mortality was 28%. The late-term results of radical operation were studied in 49 patients. Recurrent cancer developed in half (22) of the patients who underwent EPDR. Three patients who were treated by ExPDR also died from recurrent cancer and metastases. The authors believe EPDR to be expedient only in Stages I-II cancer.
