Unlabelled: Essentials Ovarian vein thrombosis (OVT) is a rare but potentially serious condition. OVT is associated with postpartum, pelvic disease, surgeries, and other thrombophilic states. We report a case of idiopathic OVT in a healthy, premenopausal woman. OVT should be considered for abdominal pain in women even outside the postpartum range.

Summary: Ovarian vein thrombosis (OVT) is a rare condition most often seen in the immediate postpartum period. We report a 40-year-old woman with no significant past medical or surgical history who presented to the emergency room for acute right lower quadrant pain of 1 day duration. A computed tomography (CT) scan showed a normal appendix but a new finding of right ovarian venous thrombosis. To date, only nine cases of idiopathic OVT have been reported. In this case report, we present a summary of these cases and review of literature regarding management of OVT.
