Background: The insulinoma associated protein tyrosine phosphatase 2 (IA-2) is one of the immunodominant autoantigens involved in the autoimmune attack to the beta-cell in Type 1 Diabetes Mellitus. In this work we have developed a complete and original process for the production and recovery of the properly folded intracellular domain of IA-2 fused to thioredoxin (TrxIA-2) in Escherichia coli GI698 and GI724 strains. We have also carried out the biochemical and immunochemical characterization of TrxIA-2and design variants of non-radiometric immunoassays for the efficient detection of IA-2 autoantibodies (IA-2A).

Results: The main findings can be summarized in the following statements: i) TrxIA-2 expression after 3 h of induction on GI724 strain yielded ≈ 10 mg of highly pure TrxIA-2/L of culture medium by a single step purification by affinity chromatography, ii) the molecular weight of TrxIA-2 (55,358 Da) could be estimated by SDS-PAGE, size exclusion chromatography and mass spectrometry, iii) TrxIA-2 was properly identified by western blot and mass spectrometric analysis of proteolytic digestions (63.25 % total coverage), iv) excellent immunochemical behavior of properly folded full TrxIA-2 was legitimized by inhibition or displacement of [S]IA-2 binding from IA-2A present in Argentinian Type 1 Diabetic patients, v) great stability over time was found under proper storage conditions and vi) low cost and environmentally harmless ELISA methods for IA-2A assessment were developed, with colorimetric or chemiluminescent detection.

Conclusions: E. coli GI724 strain emerged as a handy source of recombinant IA-2, achieving high levels of expression as a thioredoxin fusion protein, adequately validated and applicable to the development of innovative and cost-effective immunoassays for IA-2A detection in most laboratories.

Insulinomas are rare pancreatic neuroendocrine tumors (NETs) characterized by autonomous insulin secretion leading to hypoglycemia. Malignant insulinomas are defined by the presence of metastases and present significant therapeutic challenges due to limited treatment options. We report the case of a 69-year-old woman with a two-month history of neuroglycopenic symptoms, including morning headaches, blurred vision, palpitations, and sweating, which were alleviated by sugar intake.

Summary: Adrenocortical carcinoma (ACC) is a rare malignant tumour arising from the adrenal cortex, with an estimated annual incidence of one to two patients per million. Ectopic ACCs are extremely rare. The majority of ACCs are sporadic; however, ACC has been linked with genetic disease processes, including multiple endocrine neoplasia type-1 (MEN-1).

Article Synopsis
  • * A study tested radiofrequency ablation (RFA) as a less risky alternative to surgery for treating insulinomas in 29 dogs, targeting tumors in the pancreas and nearby lymph nodes/liver.
  • * Results showed RFA significantly improved blood glucose levels and reduced tumor size with minimal complications, indicating it could be a safe and effective treatment option.

Article Synopsis
  • Gastroenteropancreatic neuroendocrine tumors (GEP-NETs) can be either hormone-secreting or non-secreting, have serious health implications, and an average survival of 75-124 months.
  • Research shows that key genes involved in the tumor's development, especially epigenetic regulators like MEN1, DAXX, and ATRX, are often mutated, but their effects are not well understood.
  • The calcium sensing receptor (CaSR) is significantly reduced in GEP-NETs, potentially due to DNA methylation and chromatin modifications, suggesting it functions as a tumor suppressor by inhibiting cell growth in pancreatic NETs.

Article Synopsis
  • Type 2 diabetes is linked to IAPP fibrillation, which damages pancreatic β-cells through oxidative stress and membrane disruption.
  • Rutin, a plant polyphenol, and certain bioactive peptides (TNGQ, MANT, YMSV) show promise as inhibitors for IAPP fibrillation.
  • The study found that combining rutin with TNGQ significantly reduced IAPP fibrillation and related cell toxicity, suggesting potential for creating new anti-diabetic nutraceuticals.
