The article discusses the authors' experience in treating pyonecrotic and poorly granulating wounds of the upper and lower extremities in 158 patients; proteolytic enzymes, application of carbohydrate sorbents, and helium-neon laser radiation were used in 66 cases. With such method of treatment, the wounds healed without cutaneoplasty in 75% of cases. In 25% of cases free cutaneoplasty was performed after preparation of the wound surface. The terms of wound healing reduced 1.5-2 times as compared to that in the traditional methods of treatment.

Aim: To improve treatment of patients with grade IV chronic ischemia of lower extremities via endovascular angioplasty combined with surgical methods for suppurative-necrotic lesions of the feet.
Material And Methods: 51 patients with grade IV chronic ischemia of lower extremities underwent endovascular interventions (balloon angioplasty, stenting). A total of 23 stents were deployed in 16 patients including 12 stents in superficial and common femoral arteries, 5 in popliteal artery, 6 in iliac artery.
The microbial landscape of wounds with suppurative-necrotic complications in diabetic foot syndrome (DFS) was investigated. Capabilities by inhibiting the formation of bacterial biofilms using ultrasonic cavitation (USC) were determined. In 32 patients over the purulent necrotic complications DFS USC applied using the apparatus "Sonoca-180" company Soring (Germany), 20 patients performed a basic therapy.

Aim: Study of wound microbial landscape in suppurative necrotic complications in patients with diabetic foot syndrome (DFS), and a possibility of suppression of bacterial biofilm development in these wounds by ultrasound cavitation (USC).
Materials And Methods: 2 randomized groups of patients were included in the study: main group of 145 individuals with suppurative necrotic complications of DFS, who received ultrasound cavitation therapy, and comparison group of 86 patients who received basic therapy. In the main group Sonoca-180 (Siring, Germany) unit, that allows to combine the process of mechanical wound treatment with antibacterial action of low frequency ultrasound, was used for wound treatment.
The analysis of treatment results in 559 patients with diabetes mellitus complicated by foot pyonecrotic lesions was carried out. Surgical treatment was performed in 448 patients considering route of infection spread (basic group), conventional methods of treatment were used in 111 patients (control group). It is shown that one of the causes of unsuccessful treatment results was the infection spread from foot to leg by toes tendons and synovial sheaths.
