Purpose: HLA-B27-associated uveitis is classically thought to be a disease of the young, with a median age of onset in the 30s. There is a paucity of literature reporting on older individuals with HLA-B27 uveitis. Here, we describe cases of HLA-B27 uveitis presenting initially at age ≥70.

Methods: Retrospective review of patients with HLA-B27-associated uveitis that presented initially at ≥70 years of age.

Results: Three patients were identified. All presented with aggressive inflammation (hypopyon in two cases) and two were treated for suspected endophthalmitis. All cases were controlled sufficiently with prolonged topical anti-inflammatory therapy. One patient who was lost to follow-up demonstrated one recurrence over a 3-year period.

Conclusions: This series highlights the aggressive initial presentation of HLA-B27-associated uveitis in the older population, and demonstrates the favorable response, and limitation of recurrences, with topical immunosuppressive therapy.

HLA-B27 and spondyloarthritis: at the crossroads of innate and adaptive immunity.

Nat Rev Rheumatol

December 2024

Pediatric Translational Research Branch, National Institute of Arthritis, Musculoskeletal and Skin Diseases, National Institutes of Health, Bethesda, MD, USA.

Article Synopsis
  • HLA-B*27 significantly increases the risk of developing spondyloarthritis (SpA), which encompasses various forms including axial SpA, peripheral arthritis, and inflammation in several areas of the body.
  • The exact mechanism by which HLA-B*27 contributes to SpA is unclear, but three major hypotheses suggest it may promote autoimmune responses, activate harmful immune pathways, and lead to misfolding issues that enhance inflammation and bone formation.
  • This review highlights current theories about HLA-B*27's role in SpA, notes recent advancements in research, identifies knowledge gaps, and suggests areas for future investigation.

HLA-B*51:01 in Iranian patients with Behcet uveitis syndrome.

Reumatol Clin (Engl Ed)

November 2024

Department of Medical Genetics, School of Medicine, Tehran University of Medical Sciences, Tehran, Iran. Electronic address:

Background: Behcet's disease (BD) is a multisystem disorder prevalent along the historic Silk Road, with Behcet's uveitis (BU) representing a significant complication contributing to disability. Various studies have linked different HLA alleles with BD across diverse populations.

Methods: In this study, we investigated the association between HLA-B51:01/x and HLA-B27/x genotypes with Behcet's uveitis in 50 unrelated Iranian patients diagnosed with Behcet's uveitis, comparing them to a control group of 70 healthy individuals.

Acute anterior uveitis (AAU) is a common extra-articular manifestation of ankylosing spondylitis (AS), particularly in patients positive for the human leucocyte antigen (HLA)-B27 genetic marker. To explore the underlying mechanisms of HLA-B27 AS-associated AAU, we employed single-cell RNA sequencing to profile the transcriptomes of peripheral blood mononuclear cells in three HLA-B27 AS-associated AAU patients and three healthy controls (HCs). We identified 11 distinct immune cell clusters, with a particular focus on monocytes, revealing six subsets, including three previously unidentified subsets, namely, GTPase immune-associated proteins, Th17-related, and lncRNA monocytes, with unique gene expression patterns.

Spondyloarthritis Associated with Uveitis: A Review.

Middle East Afr J Ophthalmol

October 2024

Department of Rheumatology, Faculty of Medicine and Pharmacy, Mohammed V Military Academic Hospital, Mohammed V-Souissi University, Rabat, Morocco.

Spondyloarthritis (SpA) is a group of chronic inflammatory rheumatism characterized by common clinical, radiological, and biological manifestations occurring on a predisposing genetic background dominated by the HLA-B27 antigen. Acute anterior uveitis is the most common extra-articular feature of SpA. The objective of this review is to describe the prevalence, demographic characteristics, factors favoring the occurrence of uveitis in patients with SpA, clinical manifestations, and their therapeutic management.
