Introduction: Adrenal haemorrhage is a relatively rare condition. If there is not a specific ethology describing adrenal hematoma, then, this is termed as 'idiopathic adrenal hematoma'.
Presentation Of Case: We presented a case of idiopathic adrenal hematoma in this study. A 62-year-old woman was referred to our hospital for evaluation of a 40mm mass in the left upper abdominal cavity. The histopathological findings of the surgical specimen revealed a hematoma with normal adrenal tissue.
Discussion: The incidence of adrenal haemorrhage was found to be 1.1% regarding autopsy results. The Adrenal gland is highly vascular and vulnerable to haemorrhage. Before a surgical operation, it is difficult to diagnose idiopathic adrenal hematomas.
Conclusion: An adrenal hematoma should be kept in mind when adrenal masses assessing.

Dense-core vesicles (DCVs) are found in various types of cells, such as neurons, pancreatic β-cells, and chromaffin cells. These vesicles release transmitters, peptides, and hormones to regulate diverse functions, such as the stress response, immune response, behavior, and blood glucose levels. In traditional electron microscopy after chemical fixation, it is often reported that the dense cores occupy a portion of the vesicle towards the center and are surrounded by a clear halo.

Perioperative neurocognitive disorders (PNDs) refer to a wide spectrum of cognitive impairment persisting days to even after a year postoperative with significant morbidity and mortality. However, despite much efforts involving perioperative managements, PNDs are still prevalent with no standard preventative and therapeutic strategy. To overcome PNDs, a better understanding of pathophysiology of PNDs is crucial and a large number of studies have proven that immune-inflammatory responses from surgical stress are involved in the abnormal activation of the hypothalamic-pituitary-adrenal (HPA) axis and destabilization of neurovascular unit (NVU) that lead to PNDs.
