Fractures of the first thumb ray are common and need accurate differential treatment to restore gripping hand functions. Displaced fractures of the distal and proximal phalanx of the thumb are often treated with screws or Kirschner wires. Stable fractures can also be treated non-operatively. Fractures of the base of the first metacarpal should be differentiated into extra-articular Winterstein fractures and intra-articular Bennett or Rolando fractures. Traction forces by the abductor pollicis longus tendon regularly lead to displacement of the shaft of the first metacarpal; therefore, these fractures usually require reduction and fixation. Good functional results can be achieved by operative treatment. Fractures of the trapezium are rare. If they are displaced, operative treatment is recommended to prevent osteoarthritis of the first carpometacarpal joint.

In the medical field, endoscopic video analysis is crucial for disease diagnosis and minimally invasive surgery. The Endoscopic Foundation Models (Endo- FM) utilize large-scale self-supervised pre-training on endoscopic video data and leverage video transformer models to capture long-range spatiotemporal dependencies. However, detecting complex lesions such as gastrointestinal metaplasia (GIM) in endoscopic videos remains challenging due to unclear boundaries and indistinct features, and Endo-FM has not demonstrated good performance.

Influenza accounts for 30% of the total morbidity and mortality in the European Union. However, the specific burden in different European countries is largely unknown, and more research is needed to ascertain the reality of this disease. In this retrospective study, we analyzed the burdens of hospitalization, intensive care unit (ICU) admission and in-hospital mortality in Spain over five seasons (2015-2020) via publicly available Minimum Basic Datasets (MDBS).

Background: Medically refractory hypertonia (MRH) within the pediatric population causes severe disability and is difficult to treat. Neurosurgery for mixed MRH includes intrathecal baclofen (ITB) and lumbosacral ventral-dorsal rhizotomy (VDR). Surgical efficacy limitations can be mitigated by combining the two into a multimodal strategy.

Background: Radiation-induced sarcoma (RIS) is an exceptionally rare occurrence following radiation therapy, and manifestation usually occurs after a several-year latency period. Herein, the authors report the development of a radiation-induced osteosarcoma of the frontoparietal calvaria following treatment for an oligodendroglioma in an 84-year-old woman.
Observations: The patient had been diagnosed with a grade III anaplastic oligodendroglioma when she was 78 years old.

Background: Septic arthritis of the lumbar facet joint (SALFJ) is a rare condition that can lead to serious complications. The authors present an uncommon case in which SALFJ resulted in bacterial meningitis (BM) with hydrocephalus and pyogenic ventriculitis, causing a disturbance of consciousness. Reports describing perforation of the dura mater by an epidural abscess are rare, and the present case offers valuable insights into the management of complex and severe complications arising from SALFJ.
