The central odontogenic fibroma (COF) is a benign odontogenic tumor of mesenchymal tissue such as the periodontal ligament, dental papilla or dental follicle. It is an extremely rare tumor, accounting less than five percent of all odontogenic tumors. In this report, we describe the case of an eight year old girl who presented with a painless swelling of the left mandible. Radiographic evaluation revealed a well-defined unilocular radiolucency extending from the distal aspect of the displaced mandibular left premolar to the ascending ramus of the mandible. Histopathological examination confirmed the diagnosis of COF of the simple type. Surgical excision of the lesion was done and no recurrence was noted thereafter.
