We describe a case of rheumatic tricuspid stenosis without involvement of the mitral valve. Rheumatic disease of the tricuspid valve is very rare in the absence of concomitant mitral valvar disease. We suggest that the closing pressure at the valve is not the only factor governing localization of the rheumatic process.

Case Reports: The first case involves a 60-year-old male with a history of rheumatic heart disease and triple valve replacement who developed a high-grade AV block.

Rheumatic heart disease (RHD) affecting all four valves of the heart is extremely rare. We report quadrivalvular involvement in an early adolescent male patient. The patient presented with symptomatic severe mitral and tricuspid stenosis that underscores the rapid and fulminant nature of the disease and can still be seen in developing countries.

This study reports the challenges of rheumatic mitral and tricuspid valve re-repair on a child presenting with gross hematuria, hemolysis, and volume overload. Severe postoperative mitral and tricuspid valve regurgitation was the cause, and redo surgery for both valves resolved the acute issues.
