Clinical, immunological, liquorological, roentgenological, rheovasographic, myelographic methods and computerized tomography (CT) were used to investigate 153 patients with spondylogenic lumbosacral radiculitts. The data expanded our knowledge of pathogenesis of neurological signs of lumbar osteochondrosis. Diagnostic capacities of CT in this ailment were for the first time assessed in comparison with other investigation techniques. Clinical CT syndromes of median, paramedian and posterolateral intervertebral disc hernia were depicted. Pathogenic grounds were offered to differential use of electroneurostimulation, reflex, manual, tractional therapies in the combined treatment of the disease depending on its actual stage. CT is recommended to evaluate the treatment efficiency.

Conscious perception and unconscious effects originating from the vertebral column and its neural structures, although complex, have definite pathways represented in a network of peripheral and central nervous system (CNS) ramifications. These neural relationships consequently result in superimposed focal and diffuse, local and remote conscious perceptions and unconscious effects. Any one or combination of somatic and autonomic signs and symptoms may potentially be observed in a particular patient.

The efficacy of cytoflavin in the treatment of 60 patients (39 women, 21 men, age 32-64 year) with spondylogenic cervical and lumbosacral radiculomyeloischemia due to degenerative dystrophic spinal lesions was studied in a randomized double-blind placebo controlled study according to GCP rules. During 10 days 40 patients received intravenous cytoflavin dropper injections once daily; 20 patients (control group) received 5% glucose solution as a placebo. Considerable improvement was observed in 70% patients with radiculomyeloischemia of cervical localization and in 65%--with radiculomyeloischemia of lumbosacral segments.

The combined use of acupuncture with antidepressants was shown to enhance the effectiveness of the treatment, to increase on the average by 5-6 hours the duration of analgesia within 24 hours after each procedure, to reduce the duration of the treatment on the average by 4-5 days, that indicates the expediency of clinical use of the proposed combination.

The authors have analyzed 200 clinical cases of ischemic damage to the lower half of the spinal cord secondary to surgery-verified hernia of a lumbar intervertebral disk. In many patients the surgical findings, in addition to a compressed spinal radicle, included compression of an arterial or a venous vessel. Clinical characteristics were compared in patients with compression of the radiculo-medullar artery (n = 100) and of the large radicular vein (n = 100).
