Cardiovascular and cardiometabolic diseases are leading causes of morbidity and mortality worldwide, driven in part by chronic inflammation. Emerging research suggests that the bone marrow microenvironment, or marrow niche, plays a critical role in both immune system regulation and disease progression. The bone marrow niche is essential for maintaining hematopoietic stem cells (HSCs) and orchestrating hematopoiesis.

This case report highlights the complex clinical presentation of a 43-year-old male with a history of renal transplantation, hypertension, and diabetes mellitus, who developed viral meningitis with probable metabolic encephalopathy. The multidisciplinary approach involved infectious disease specialists, transplant nephrologists, and neurologists. This case provides unique learning points such as highlighting the complexities of diagnosing and managing viral meningitis in an immunocompromised post-transplant patient, emphasizing the importance of a multidisciplinary approach, innovative medication delivery, and awareness for complications such as metabolic encephalopathy and persistent fevers.

Type 2 diabetes (T2D) is a long-term metabolic condition that presents considerable health challenges globally. As the disease progresses, the interplay between genetic, environmental, and lifestyle factors becomes increasingly evident, leading to complications. Epigenetics has emerged as a critical area of research, providing insights into how these factors can modify the expression and cellular behavior without altering the underlying DNA sequence.
