This case demonstrates a previously unreported congenital orbital deformity. The patient was born with a unilateral exophthalmos. The etiology of the defect was demonstrated by CT scan as a convex bowing of the right superomedial orbital wall behind the axis of the eye. At surgery, this convexity was revealed to be a small bony defect through which the periorbital tissue and dura mater adhered. This sort of orbital deformity, although subtle, must be considered in the differential diagnosis of congenital unilateral exophthalmos.

Background: Hemifacial microsomia (HFM) presentation includes gross distorted ramus, malposition temporomandibular joint, small glenoid fossa, distorted condyle and notch, malformed orbit, cupping ear or absent external ear, and facial nerve palsy. HFM is the second most prevalent congenital deformity of the face, with little literature from the South African population. This retrospective study elucidated the demographic characteristics and clinical presentations of HFM patients in a select South African population and compared it to the literature.

Purpose: The purpose of this manuscript is to report a rare case of an orbital cyst detected intrauterine with sonography.
Observation: A 23-year-old female presented for routine prenatal monitoring when an orbital cyst was detected with a transabdominal ultrasound. Uncomplicated cesarean section was performed at 38 weeks gestation with proptosis of the left globe being noted on ophthalmic examination of the newborn.
