Objective: To estimate the prevalence of Mycoplasma genitalium in Toronto, Ont; detect mutations associated with macrolide and fluoroquinolone resistance; and describe treatment outcomes.
Design: Prospective, cross-sectional study.
Setting: A sexual health clinic in Toronto.
Participants: A consecutive sample of men and women attending the sexual health clinic between September 1, 2013, and December 20, 2013.
Interventions: Participants underwent testing for M genitalium, along with standard sexually transmitted infection screening. All samples that had positive results for M genitalium were tested for mutations associated with resistance to macrolides and fluoroquinolones. Mycoplasma genitalium treatment was based on resistance profile and verified with a test of cure.
Main Outcome Measures: Positive results for M genitalium and antibiotic resistance.

Background: Recurrent vaginal discharge is an important cause of repeated visits to the gynaecologists. Failure to correctly identify the causative organism with standard microscopy and culture techniques results in repeated unsuccessful treatment and the risk of developing antibiotic resistance. Multiplex PCR test is increasingly being used for investigating infections where multiple organisms may be involved.

A 30-year-old male patient with symptomatic Mycoplasma genitalium urethritis failed treatment with oral azithromycin, 2-stage doxycycline-moxifloxacin, and minocycline. Molecular testing confirmed the presence of macrolide resistance mutations. Treatment with oral tinidazole 2 g daily for 7 days resulted in clinical and microbiologic cure.

Proctitis refers to inflammation in the rectum and may result in rectal bleeding, discharge, urgency, tenesmus, and lower abdominal pain. It is a common presentation, particularly in genitourinary medicine and gastroenterology, as the two most common causes are sexually transmitted infections and inflammatory bowel disease. The incidence of infective proctitis is rising, particularly amongst high-risk groups, including men who have sex with men, those with HIV seropositive status, and those participating in high-risk sexual behaviours.
