Ewing sarcoma/primitive neuroectodermal tumor (ES/PNET) of the kidney is extremely rare, and is usually diagnosed after nephrectomy without neoadjuvant chemotherapy. Although ifosfamide and etoposide improve survival to a great extent in ES/PNET, the use of nephrotoxic agent, particularly ifosfamide, is a concern after nephrectomy. We describe the case of a 14-year-old female patient with abdominal mass who was diagnosed with ES/PNET of the right kidney after nephrectomy. Adjuvant chemotherapy including ifosfamide and etoposide were given. The estimated glomerular filtration rate decreased to 75% after the end of therapy. There was no evidence of recurrence 70 months after initial diagnosis.

Primary extraosseous Ewing sarcomas have also been described in several other organs and locations other than bones, including the pancreas. Desmoplastic small round cell tumors (DSRCTs) of the gastrointestinal (GI) tract are a rare and highly aggressive variant of soft tissue sarcomas, predominantly affecting the abdominal region. These tumors are believed to originate from multipotent mesenchymal stem cells or primitive progenitor cells. They are composed of small round tumor cells associated with prominent stromal desmoplasia, polyphenotypic differentiation, and gene fusion.

Extraosseous Ewing's sarcoma (EES) is a rare form of Ewing's sarcoma that arises outside the bones in soft-tissue structures. It is thought to result from a genetic abnormality involving the fusion of the EWSR1 gene with various partner genes, most commonly the FLI-1 gene. Common symptoms include pain, swelling, and sometimes a palpable mass at the site of the tumor.
