A 46 year-old man with acquired immunodeficiency syndrome presented with sudden development of vertigo and tinnitus and then simultaneous, bilateral, profound, sudden hearing loss. Magnetic resonance imaging showed bilateral high signal within the cochlea, vestibule, and portions of the semicircular canals on the non-enhanced T1-weighted images, most consistent with recent hemorrhage into the otic labyrinth. Serum analysis and bone-marrow biopsy led to diagnosis of Waldenstrom's macroglobulinemia - a likely cause of the presumed hemorrhage.

Purpose: The neuropathologic mechanisms of sudden sensorineural hearing loss (SSNHL) are unknown. The aim of this study was to investigate the alterations of neurovascular coupling (NVC) in patients with SSNHL and its association with hematologic inflammatory factors.
Methods: The amplitude of low-frequency fluctuations (ALFF), fractional amplitude of low-frequency fluctuations (fALFF), regional homogeneity (ReHo), and degree centrality (DC) were calculated in 48 patients with SSNHL and 54 age-, gender-, and education-matched healthy control (HC), and also utilized the arterial spin labeling imaging (ASL) to calculate cerebral blood flow (CBF).
Sudden sensorineural hearing loss (SSNHL) presents as the abrupt onset of hearing loss. Approximately 88% of SSNHL has no identifiable etiology and is termed idiopathic sudden sensorineural hearing loss (ISSHL). Hearing specialists have investigated ISSHL since the 1970s.
