Background: Cavernous malformations (CMs) arising in the cranial nerves are rare. Only a few cases of facial nerve CMs have been reported in the literature.
Case Description: A 52-year old woman presented with headache, unilateral worsening facial weakness, and hearing loss. Imaging was suggestive of a CM of the cerebellopontine angle. Intraoperatively, a mulberry-shaped lesion was seen arising entirely within the seventh nerve. The patient underwent complete resection with sural nerve interposition graft.
Conclusions: CMs are very rare but should be included in the differential diagnosis of cerebellopontine angle masses.

DICER1-associated sarcoma is an emerging entity, defined by either somatic or germline dicer 1, ribonuclease III (DICER1) mutations and sharing characteristic morphologic features irrespective of the site of origin. In addition to the DICER1 driver mutation, concurrent genomic alterations, including tumor protein 53 (TP53) inactivation and RAS pathway activation, are frequently detected. Tumors that morphologically resemble malignant peripheral nerve sheath tumor (MPNST) have rarely been reported among DICER1 sarcomas and often pose diagnostic challenges.

Microvascular decompression (MVD) is a neurosurgical operation used to treat trigeminal neuralgia (TN). The surgery is performed through a retrosigmoid approach, where a Teflon pledget is placed in between the offending vessel (most commonly the superior cerebellar artery) and trigeminal nerve. The surgery is performed within the superior aspect of the cerebellopontine angle (CPA) through a small working corridor that is triangulated by the petrous bone and tentorium.
