Appendiceal mucoceles are rare lesions with a variable clinical presentation often identified incidentally on imaging or at laparotomy/laparoscopy for an unrelated diagnosis. Mucocele of the appendix may be a benign or malignant process, making early recognition based on symptoms and key radiographic characteristics of the utmost importance for optimal patient management. Here we present the case of a patient presenting with non-specific abdominal complaints suffering from appendiceal mucocele perforation due to low-grade mucinous adenocarcinoma.

An appendiceal mucocele is a rare clinical entity often mimicking that of acute appendicitis in a majority of cases leading to incidental intraoperative findings. Nevertheless, appropriate diagnosis is vital prior to surgery to prevent complications such as pseudomyxoma peritonei. This report details a case of a man in his 70s, who was admitted to the emergency department with right iliac fossa pain with imaging showing wall thickening of the caecum and the ascending colon.

Appendiceal neoplasms are usually asymptomatic or associated with mild, nonspecific symptoms. Due to the rarity of the disease and the lack of specific symptoms, this clinical entity escapes the diagnostic consideration of the gynecologist, when women come in with right iliac fossa pain. A case is presented of a 56-year-old woman with a mass in the right small pelvis, which was preoperatively diagnosed as originating from the ovary.

Appendiceal mucocele is a rare entity first described by Carl von Rokitansky, characterized by cystic dilatation of the appendiceal lumen due to obstruction, epithelial proliferation or inflammation and accumulation of mucoid material. The cause can be either neoplastic or non-neoplastic. Patients with appendiceal mucocele can be asymptomatic or present with right lower quadrant pain which may mimic acute appendicitis.
