Case report: Study of a bulky melanoma mimicking sarcoma.

Int J Surg Case Rep

January 2025

Department of Head and Neck Surgery, Institut de Cancérologie de Lorraine, 6 avenue de Bourgogne, 54519 Vandœuvre-lès-Nancy, France; CRAN, CNRS, UMR 7039, Université de Lorraine, Vandœuvre-lès-Nancy, France; Faculté d'odontologie de Lorraine, Université de Lorraine, Vandœuvre-lès-Nancy, France.

Introduction: Large melanomas, while relatively uncommon, present significant diagnostic challenges due to their size and potential to mimic other malignancies, leading to delays in appropriate treatment. Initial misdiagnosis is a substantial concern, impacting patient outcomes. This case highlights the importance of immunohistochemistry in cancer diagnosis, and of appropriate therapeutic management, which here included excision surgery of the tumor mass.

The evolution of regional anesthesia techniques has markedly influenced the management of postoperative pain, particularly in thoracic surgery. As part of a multimodal analgesic approach, fascial plane blocks have gained prominence due to their efficacy in providing targeted analgesia with minimal systemic side effects. Among these, the superficial intercostal plane (SPIP) block and deep parasternal intercostal plane (DPIP) block are of notable interest.

The anatomy of the sciatic nerve allows it to be blocked at different levels using various anesthetic approaches. However, for several reasons, performing these approaches may be challenging or disadvantageous in specific categories of patients, particularly in obese patients. The objective of this brief technical report is to describe a new technical approach to sciatic nerve block, designed to simplify the procedure for certain categories of patients and less experienced practitioners.

Background: Primary tumor thickness is important for prognosis of melanoma patients. To enhance prevention and quantify the true burden of melanoma, better understanding of thickness patterns and associated characteristics is crucial. Previous studies have been limited to report trends and address risk factors of thickness in specific melanoma subtypes in the Greek population.

DeepFocus: a transnasal approach for optimized deep brain stimulation of reward circuit nodes.

J Neural Eng

January 2025

Department of Electrical and Computer Engineering, Carnegie Mellon University, 5000 Forbes Avenue, Pittsburgh, Pennsylvania, 15213-3815, UNITED STATES.

Objective: Transcranial electrical stimulation (TES) is an effective technique to modulate brain activity and treat diseases. However, TES is primarily used to stimulate superficial brain regions and is unable to reach deeper targets. The spread of injected currents in the head is affected by volume conduction and the additional spreading of currents as they move through head layers with different conductivities, as is discussed in [1].
