Background: For a growing number of food-based dietary guidelines (FBDGs), diet optimization is the tool of choice to account for the complex demands of healthy and sustainable diets. However, decisions about such optimization models' parameters are rarely reported nor systematically studied.
Objectives: The objectives were to develop a framework for (i) the formulation of decision variables based on a hierarchical food classification system; (ii) the mathematical form of the objective function; and (iii) approaches to incorporate nutrient goals.

Context: This paper aims to review the findings of the Tehran Obesity Treatment Study (TOTS) on obesity and bariatric surgery (BS).
Evidence Acquisition: The objective of this review is to assess all aspects of BS in individuals with severe obesity, focusing on research conducted within the TOTS framework.
Results And Conclusions: The TOTS studies have produced significant national-level findings, highlighting critical issues related to the effectiveness and outcomes of bariatric procedures, the importance of comprehensive nutritional management, and the complications associated with these interventions in this population.

Decentralised study procedures address some of the recruitment and retention limitations in traditional trial methodology. Acute intermittent porphyria (AIP) is a rare metabolic disorder characterized by acute attacks often triggered by porphyrinogenic drugs and a low-glucose diet. According to recent findings, chronic symptoms persist in AIP patients. To avoid the symptoms, patients often adopt preventive strategies such as increasing glucose intake, suggesting that nutrition is a crucial aspect of disease management.
