The present meta-analysis pooled the results from randomized controlled trials (RCTs) to identify and assess the efficacy and safety of thrombin-based hemostatic agent in primary total knee arthroplasty (TKA). Potential academic articles were identified from the Cochrane Library, Medline (1966-2015.5), PubMed (1966-2015.5), Embase (1980-2015.5), and ScienceDirect (1966-2015.5). Relevant journals and the recommendations of expert panels were also searched by using Google search engine. RCTs assessing the efficacy and safety of thrombin-based hemostatic agent in primary TKA were included. Pooling of data was analyzed by RevMan 5.1 (The Cochrane Collaboration, Oxford, UK). A total of four RCTs met the inclusion criteria. The meta-analysis revealed significant differences in postoperative hemoglobin decline ( < 0.00001), total blood loss ( < 0.00001), drainage volume ( = 0.01), and allogenic blood transfusion ( = 0.01) between the treatment group and the control group. No significant differences were found regarding incidence of infection ( = 0.45) and deep vein thrombosis (DVT;  = 0.80) between the groups. Meta-analysis indicated that the application of thrombin-based hemostatic agent before wound closure decreased postoperative hemoglobin decline, drainage volume, total blood loss, and transfusion rate and did not increase the risk of infection, DVT, or other complications. Therefore, the reviewers believe that thrombin-based hemostatic agent is effective and safe in primary TKA.

Abnormal levels of thrombin may be associated with various diseases, such as thrombosis and hemorrhagic diseases, making precise detection of thrombin particularly important. Dual signal detection is a method that enhances detection sensitivity and specificity by simultaneously utilizing two different signals. Its primary advantages include improving detection accuracy and reducing false positive rates, making it particularly suitable for clinical analysis and diagnostics.

Article Synopsis
  • This retrospective study aimed to identify risk factors for allogeneic red blood cell transfusion in adult spinal deformity surgery by reviewing the medical records of 151 patients who underwent corrective surgery from 2012 to 2021.
  • Key factors examined included intraoperative hypothermia, autologous blood donation, and the use of hemostatic agents, which have been underexplored in previous research.
  • The findings revealed that not using TXA, lacking autologous blood donation, and having longer surgery times were significant predictors of needing blood transfusions during the perioperative period.

As one of the biomarkers of coagulation system-related diseases, the detection of thrombin is of practical importance. Thus, this study developed a portable biosensor based on a personal glucometer for rapid detection of thrombin activity. Fibrinogen was used for the detection of thrombin, and the assay principle was inspired by the blood coagulation process, where thrombin hydrolyzes fibrinogen to produce a fibrin hydrogel, and the amount of invertase encapsulated in the fibrin hydrogel fluctuates in accordance with the activity of thrombin in the sample solution.

Background: Prolonged bleeding from arteriovenous fistulas (AVF) and arteriovenous grafts (AVG) associates with worse outcomes; Within the hemodialysis unit these outcomes include anemia and quality of life disruptions, and outside the hemodialysis unit includes fatal hemorrhage. However, various guidelines for AVF/AVG bleeding management inside and outside the hemodialysis unit lack consensus.

Methods: A scoping review was conducted of four databases, from inception to 17 February 2024.

Prosthetic heart valve (PHV) replacement has increased the survival rate and quality of life for heart valve-diseased patients. However, PHV thrombosis remains a critical problem associated with these procedures. To better understand the PHV flow-related thrombosis problem, appropriate experimental models need to be developed.
