IgG4-related disease (IgG4-RD) is a systemic inflammatory condition characterized by tissue infiltration with IgG4-positive plasma cells and a tendency to form mass-like lesions in various organs. IgG4-related sinusitis, although a relatively rare manifestation of IgG4-RD, significantly impacts the paranasal sinuses. A 52-year-old man presented with persistent rhinorrhea, nasal obstruction, and headaches.

Objectives: This study aimed to assess (1) effects of abatacept on salivary gland histology of Sjögren's disease (SjD) patients, (2) the predictive value of salivary gland histopathological characteristics at baseline for clinical response to abatacept treatment.
Methods: Patients (n=41) who participated in the Dutch ASAP-II and ASAP-III trials and international abatacept trial (IM101603) from whom a labial (n=13) or parotid (n=28) salivary gland biopsy was obtained at baseline and after 24 weeks of treatment with abatacept were included. Biopsies were analysed for SjD related histopathological features before and after abatacept (n=25) or placebo (n=16) treatment.

A teenage girl who had recently begun playing saxophone presented with recurrent painless right cheek swelling for six months. Despite several courses of antibiotics, the swelling continued to re-occur. On examination, enlargement of the right parotid gland and crepitus over the same region are noted, with no signs of acute inflammation.
View Article and Find Full Text PDFPleomorphic adenoma (PA) is a benign salivary gland tumor with diverse cytomorphological and architectural features, typically presenting biphasic ductal structures within a chondromyxoid matrix. We report a unique case of PA where dense lymphoid infiltration disrupted these structures, resulting in duct-like slit structures lined with a single layer of spindle cells, lacking the biphasic pattern. These spindle cells demonstrated myoepithelial nature, confirmed by positive immunostaining for pan-cytokeratin, S100 protein, and calponin.

We report a case of IgG4-related dacryoadenitis and sialadenitis (Mikulicz's disease) with acquired hemophilia A (AHA) in a pediatric patient. An 11-year-old female presented with intermittent swelling of the bilateral upper eyelids and neck areas. Clinical examination revealed bilateral swollen upper eyelids with palpable mass lesions in the lacrimal fossa.
