Acute ischemic stroke, a medical emergency caused by reduced cerebral blood flow, results in brain cell damage. While commonly associated with older individuals, strokes can also occur in young and middle-aged adults, posing significant socio-economic and health challenges due to the long-term impact of the condition. Introduction: Extraskeletal myxoid chondrosarcoma (EMC) is a rare sarcoma defined by NR4A3 gene rearrangements, typically featuring uniform cells with eosinophilic cytoplasm and mild atypia, arranged in cords or clusters within a chondromyxoid stroma. A cellular variant, characterized by increased cellular density and a solid growth pattern, has been recognized.
Methods: We encountered three cases of round cell sarcomas, diagnosed as EMC based on NR4A3 or NR4A2 rearrangements.

A 77-year-old man with multiple comorbidities presented with cough, dyspnea and nonspecific malaise. Chest computed tomography revealed a mass in the right lower lobe of the lung, along with an associated hypodense lesion in the left atrium. Echocardiography showed a mobile mass in the left atrium, initially suspected to be a thrombus.

Synovial sarcoma is a rare malignant mesenchymal neoplasm primarily affecting adolescents and young adults, and it typically arises from deep soft tissues near large joints. Although commonly found in extremities, it can occur in various anatomic locations. We present a rare case of a 29-year-old man with primary pulmonary synovial sarcoma manifesting as a cystic mass masquerading as a lung hydatid cyst.
