Unlabelled: Tibiotalocalcaneal arthrodesis with an intramedullary nail is a procedure reserved for patients who have conditions affecting the ankle joint and subtalar joint. The most common complications include nonunion, malunion, delayed union, infection, periprosthetic fracture, hardware failure, lateral plantar nerve injury, and wound healing. Significant bone void may result if the hardware requires removal secondary to one of these complications. The purpose of this article is to report on 2 cases of infected intramedullary nails, which were revised with fibular inlay strut grafts. Both cases were augmented with either internal or external fixation.

Levels Of Evidence: Therapeutic, Level IV: Case report.

This case report presents a three-year follow-up of a young male with bilateral hindfoot fractures due to a high-energy road traffic accident, resulting in a comminuted open calcaneal fracture on the left and an open fracture-dislocation of the right talus. Staged reconstruction was performed, including initial debridement, temporary cement spacers, and subsequent fixation with femoral head allografts (FHAs). The right foot underwent a tibiotalocalcaneal (TTC) fusion, and the left foot received a double arthrodesis.

Tibiotalocalcaneal (TTC) fusion with an intramedullary nail (IMN) has been utilized for a myriad of indications in hindfoot and ankle reconstruction. However, some controversies remain on the optimal position of the hindfoot. Previous studies have reported on the potential medialization of the rearfoot during insertion of the IMN, but few studies have examined the potential affect on the subtalar joint.

Ankle dislocations, particularly those that are old and neglected, pose significant challenges in orthopedic management due to the development of arthritic changes and surgical difficulties of reduction. The optimal treatment often involves stabilization and realignment to ensure proper healing. The closed reduction is rarely achieved in old neglected dislocations.
