Background: Revision total hip arthroplasty is a technically demanding procedure which has gained importance for more than two decades. It was a nightmare for revision surgeons during its initial years of inception before the advent of extended trochanteric osteotomy (ETO). This technique gains access to the femoral medullary canal without compromising the bone stock and aids removal of primary implant and cement mantle without further damaging the parent bone. Like any other surgery, ETO does have certain limitations and complications as reported by various authors. Though it has been routinely used by revision surgeons, thorough knowledge of technical details of ETO is still lacking. So this review article is aimed at addressing the indications, surgical procedure, fixation technique, implant selection and complication of ETO which has been presented over a period of years by various authors.

Methods: We searched in the most commonly used portals like MEDLINE (PubMed) and Google scholar using appropriate terminologies for the literature regarding the various preoperative, intraoperative and postoperative clinical scenarios in which revision surgeons utilized ETO.

Results And Conclusion: ETO is an important tool in the revision surgeon's armamentarium and can be used in variety of clinical scenarios and for various intraoperative needs and goals. Awareness about biomechanics of ETO, indications, implants selection, fixation techniques and complications is paramount for good intraoperative and postoperative outcome. ETO by posterior approach continues to be a work horse approach for most revision surgeons all over the world.

An effective surgical educational system in the era of robotic surgery: "Double-Surgeon Technique" in robotic gastrectomy for minimally invasive surgery.

Langenbecks Arch Surg

December 2024

Department of Gastroenterological Surgery, Okayama University Graduate School of Medicine, Dentistry and Pharmaceutical Sciences, 2-5-1 Shikata-Cho, Kita-Ku, Okayama City, Okayama, 700-8558, Japan.

Purpose: Gastric cancer (GC) remains a major malignancy. Robotic gastrectomy (RG) has gained popularity due to various advantages. Despite those advantages, many hospitals lack the necessary equipment for RG and are still performing laparoscopic gastrectomy (LG) due to its established minimal invasiveness and safety.

Background: Traditional decannulation of femoral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) involves femoral cutdown. Percutaneous methods have been developed, but data supporting their use is limited. We sought to compare the MANTA vascular closure device to open decannulation.

Importance: Intraoperative parathyroid hormone (IOPTH) monitoring is recommended by the American Association of Endocrine Surgeons for use during parathyroidectomy for patients with primary hyperparathyroidism (PHPT), but there is no clinician consensus regarding the IOPTH monitoring criteria that optimize diagnostic accuracy.

Objective: To evaluate and rank the diagnostic properties of IOPTH monitoring criteria used during surgery for patients with PHPT.

Data Sources: A bayesian diagnostic test accuracy network meta-analysis (DTA-NMA) was performed, in which peer-reviewed citations from January 1, 1990, to July 22, 2023, were searched for in MEDLINE, Embase, Web of Science, CENTRAL, and CINAHL.

Background: Is "eyeballing" enough to determine cup malposition on anteroposterior (AP) pelvis radiographs before revision total hip arthroplasty (rTHA) for instability? We aimed to determine the following: (1) the reliability of eyeballing cup inclination/anteversion on AP pelvis radiographs vs geometrical measurements and (2) whether visual assessments are affected by surgeon experience.

Materials And Methods: Fifteen de-identified standing AP pelvis radiographs obtained before rTHA for instability were evaluated by one orthopedic surgeon who measured inclination/anteversion of the cups (n=15) using a new simplified method based on basic geometry. Subsequently, 4 orthopedic surgeons and 4 fellows (postgraduate year 6) assessed inclination/anteversion by eyeballing.

The Japanese Society of Oral Oncology and Japanese Society of Oral and Maxillofacial Surgeons have jointly developed clinical practice guidelines for oral cancer (oral squamous cell carcinoma) to improve and standardize the quality of oral cancer treatment in Japan. The first, second, and third editions were published in 2009, 2013, and 2019, respectively, and the 2023 edition was recently developed. In the development of the 2023 edition, 60 clinical questions (CQs) were listed.
