Zoledronic acid is a parenteral long-acting bisphosphonate that has been shown to be more effective than other bisphosphonates in treating hypercalcemia of malignancy. It is important to be aware of its ability to induce prolonged and severe hypocalcemia (hypoCa) following administration, which can be difficult to control despite aggressive calcium replacement. We report on a patient with metastatic breast cancer who presented with severe symptomatic hypoCa after receiving zoledronic acid for hypercalcemia of malignancy.

SummaryWe report a case of small cell carcinoma of the ovary, hypercalcaemic type, a rare and aggressive form of ovarian cancer, causing an acute kidney injury. A woman in her mid-30s presented with a large pelvic mass and abdominal distention, this was associated with rapidly deteriorating renal function, which did not improve with standardised kidney injury treatment. There was a high suspicion of ovarian cancer.

Fever of unknown origin (FUO) can be a common manifestation of multiple disease processes like infections, hematological & solid organ malignancies, autoimmune disorders, and autoinflammatory diseases. Endocrine causes of FUO are rare but should be considered in differential diagnosis. We present a case of a 35-year-old female with prolonged on-and-off fever and intermittent vomiting for nine months, where extensive workups for chronic infections, malignancy, and autoimmune conditions initially yielded no definitive diagnosis.
