Increasingly, care for elderly individuals with chronic diseases who take multiple medications is being provided in community-based settings. Pharmacists and other health care providers are responding to this trend by collaborating in new, innovative ways such as practicing in physicians' offices or partnering with community pharmacies. The goal is to care for these elderly patients and help them effectively manage their medications where they live. These changes are having another result-they are transforming what patients, insurers, and other health providers expect from pharmacists who are helping to manage the medications and health care of older adults.
