Background: Computer-assisted planning and intraoperative implementation using templates have become appreciated modalities in craniofacial reconstruction with fibula and DCIA flaps due to saving in operation time, improved accuracy of osteotomies and easy insetting. Up to now, a similar development for flaps from the subscapular vascular system, namely the lateral scapular border and tip, has not been addressed in the literature.

Patients/method: A cohort of 12 patients who underwent mandibular (n = 10) or maxillary (n = 2) reconstruction with free flaps containing the lateral scapular border and tip using computer-assisted planning, stereolithography (STL) models and selective laser sintered (SLS) templates for bone contouring and sub-segmentation osteotomies was reviewed focussing on iterations in the design of computer generated tools and templates.

Results: The technical evolution migrated from hybrid STL models over SLS templates for cut out as well as sub-segmentation with a uniplanar framework to plug-on tandem template assemblies providing a biplanar access for the in toto cut out from the posterior aspect in succession with contouring into sub-segments from the medial side.

Conclusion: The latest design version is the proof of concept that virtual planning of bone flaps from the lateral scapular border can be successfully transferred into surgery by appropriate templates.

Defining a danger zone for iatrogenic long thoracic nerve injury in gender-affirming mastectomy.

J Plast Reconstr Aesthet Surg

November 2024

Oregon Health and Science University, Division of Plastic and Reconstructive Surgery, Portland, OR, USA; Oregon Health and Science University, Transgender Health Program, Portland, OR, USA. Electronic address:

The long thoracic nerve's (LTN) superficial location on the chest wall renders it vulnerable to iatrogenic injury. Plastic surgeons' gender-affirming mastectomy volumes are rapidly increasing. This operation involves lateral chest contouring placing the distal LTN at risk of injury along the chest wall.

Background: Scapular Dyskinesia (SD) is detected more frequently in athletes who do overhead activity sports. SD is more common in kickboxers who actively use their upper extremities. This study was conducted to compare upper extremity stabilization, proprioception and body image in kickboxers with and without SD.

Far Posterior Approach for Rib Fracture Fixation: Surgical Technique and Tips.

JBJS Essent Surg Tech

December 2024

Department of Orthopedics, OhioHealth Health System, Columbus, Ohio.

Article Synopsis
  • The video article discusses the far posterior or paraspinal approach for treating posterior rib fractures, which enhances intraoperative visibility and minimizes muscle damage.
  • This method helps preserve periscapular strength, achieving up to 95% recovery six months after surgery through muscle-sparing techniques.
  • The surgical process involves precise skin incision and careful dissection of muscles like the trapezius, rhomboids, and latissimus dorsi to allow for effective access and treatment of the fractures without significant tissue loss.

Humeral Stem Design in Reverse Total Shoulder Arthroplasty.

Curr Rev Musculoskelet Med

December 2024

Department of Orthopaedic Surgery, University of California San Francisco, 500 Parnassus Ave MU-320W, San Francisco, CA, 94143, USA.

Purpose Of The Review: There have been tremendous modifications to the humeral component since Paul Grammont first introduced the reverse total shoulder arthroplasty in 1985. The purpose of this article is to review historical design features and their drawbacks and to summarize the clinical outcomes of modern designs.

Recent Findings: Decreasing the neck-shaft angle and increasing humeral lateralization have helped address problems of scapular notching and limited internal and external rotation that were common with traditional designs.

Introduction: This article presents a simultaneous operative technique, which allowed to successfully treat a Cuff Tear Arthropathy (CTA), instability and Levy-type III scapular spine fracture non-union with 90°-double plating, humerus-to-scapula grafting and simultaneous reverse shoulder arthroplasty.

Case Report: We present a 64year-old woman with previously known cuff arthropathy (2012/53yo). She fell in 2017 (58 years old with minor shoulder disability) before dislocating the shoulder in 2020 (60 years old) with evidence of a medial Levy-type III scapular spine fracture non-union and severe apprehension, unable to anterior elevate her arm more than 90°.
