Background: Propofol is widely used in sedation for colonoscopy, but its adverse effects on cardiovascular and respiratory systems are still concerning. The present study investigated whether target controlled infusion (TCI) of propofol could provide a better sedation quality than manually controlled infusion (MCI) in training inexperienced anesthesiology residents.
Material/methods: Eighteen training residents were allocated into 2 groups receiving TCI and MCI training in their first month in the endoscopy center, while receiving MCI and TCI training instead in their second month. The last 2 patients at the end of each month were included to analyze the sedation quality of TCI and MCI techniques by comparing satisfaction of endoscopist and patients based on the visual analogue scale (VAS). Heart rate (HR), mean blood pressure (MAP), SpO2, and recovery time were also compared as the secondary outcomes.
Results: The demographic data were similarly distributed among the TCI and MCI patients. Endoscopist's satisfaction score in the TCI group was significantly higher than in the MCI group, 81.3±7.2 versus 74.2±9.5 (P=0.003), but the patients' satisfaction score was similar between the 2 groups. More stable hemodynamic status was obtained in the TCI group, manifested as higher lowest MAP and lower highest MAP than in the MCI group. Lowest SpO2 in the TCI group was significantly higher than in the MCI group. Patients in the TCI group recovered earlier than in the MCI group.
Conclusions: TCI is a more effective and safer technique for anesthesiology residents in sedation for colonoscopy.

Importance: The closed-loop infusion system can automatically adjust and maintain the depth of anesthesia by using the propofol target-controlled infusion (TCI) model under the feedback guidance of the bispectral index (BIS).
Objective: To evaluate the safety and superiority of closed-loop TCI of propofol guided by BIS during maintenance of generalized intravenous anesthesia for preschool children.
Methods: A total of 120 children aged 1-6 years were enrolled and were divided into a closed-loop feedback group (Group C) and an open-loop manual control group (Group O), with 60 participants in each group.

Aim: Severe painful neuropathy often occurs in cancer patients receiving chemotherapy. Emerging evidence has demonstrated that microglia contribute to the occurrence and development of cancer-associated pain. This study aimed to investigate the mechanisms by which piperine influences cancer-associated pain induced by microglia activation.

North-Western section of Bangladesh is experiencing a protracted decrease in precipitation, irregular rainfall, and the depletion of ground water, which results in water scarcity and extreme dry weather that impedes the production of agricultural commodities and threatens the people's way of life. Analyzing the precipitation deficit and ground water deficit, along with vegetation cover, temperature condition, and the condition of the vegetation is a crucial component of drought vulnerability assessment. Rajshahi Zilla, a region of Bangladesh located in the middle of the Barind tract, is experiencing a severe water shortage.

Background: Measures that quantify clinician fulfillment and engagement with work (clinician experience measures) have the potential to improve the quality, effectiveness, and enjoyment of patient care. Many existing measures of clinician fulfillment and engagement with work are relatively long and potentially burdensome, negatively framed, or address personal well-being. A measure with a small number of items that address positive attributes to which a team aspires may be a more useful measure to guide organizational improvement efforts.
