Objective: To evaluate the impact of screening hyper and hypoglycemia measured by capillary glycemia and standard monitorization of hyperglycemic patients hospitalized in regular care units of Hospital Israelita Albert Einstein.

Methods: The capillary glycemia was measured by the Precision PCx (Abbott) glucosimeter, using the PrecisionWeb (Abbott) software. The detection of hyper and hypoglycemia during the months of May/June were compared to those of March/April in 2009 and to the frequency of the diagnosis of diabetes in 2007.

Results: There was an increase in the glycemia screening from 27.7 to 77.5% of hospitalized patients (p < 0.001), of hyperglycemia detection (from 9.3 to 12.2%; p < 0.001) and of hypoglycemia (from 1.5 to 3.3%; p < 0.001) during the months of May/June 2009. According to this action 14 patients for each additional case of hyperglycemia and 26 cases for each case of hypoglycemia were identified. The detection of hyperglycemia was significantly higher (p < 0.001) than the frequency of registered diagnosis related do diabetes in the year of 2007.

Conclusions: the adoption of an institutional program of glycemia monitorization improves the detection of hyper and hypoglycemia and glycemia control in hospitalized patients in regular care units.

Association of early dysglycemia with intraventricular hemorrhage and mortality in very low birth weight infants.

Eur J Pediatr

December 2024

Clinical Pharmacy Department, Pharmaceutical Care Services, King Saud Medical City, Riyadh, Saudi Arabia.

Article Synopsis
  • The study examines the combined effects of hyperglycemia and hypoglycemia on intraventricular hemorrhage (IVH) and mortality in very preterm infants admitted to a neonatal intensive care unit from January 2020 to January 2024.
  • Out of 554 infants analyzed, 75.5% experienced hyperglycemia and/or hypoglycemia in their first postnatal week, with IVH occurring in 28.5% of infants and 13.7% resulting in death during the study.
  • The findings indicate that both isolated hyperglycemia and the combination of hypo- and hyperglycemia significantly increase the risk of IVH and correlate with higher mortality rates among these infants.

Background: During calendar years 2021 through 2023, our health system admitted 200,837 persons 18 years and older with diabetes, of which 61% (n = 123,393) were 65 years and older with orders for insulin administration. The incidence of diabetes among hospitalized persons 65 and older continues to increase in the United States, with 24 million adults 65 and older with diabetes reported in 2020. Insulin, a high-risk medication, has the potential for adverse drug events, which can cause significant harm to patients, potentially resulting in death.

Article Synopsis
  • Dysglycemia is a common issue in severe sepsis and can indicate poor patient outcomes, but there’s limited research specifically on non-diabetic patients experiencing this condition during sepsis episodes.
  • This study, conducted at a medical college hospital in Kochi from January to December 2023, aimed to examine the prevalence of dysglycemia among 100 non-diabetic sepsis patients and analyze its links to factors like age, gender, APACHE II scores, and the length of hospital stay.
  • Findings revealed that 49% of included patients had dysglycemia (16% had hypoglycemia and 33% had hyperglycemia), with a statistically significant increase in incidence as age increased (p=0.002).

Article Synopsis
  • This study examines the clinical features and genetic aspects of Congenital Bile Acid Synthesis Disorder Type 3 (BASD3), specifically related to mutations in the CYP7B1 gene, using two case studies from hospitals in Xiamen, China, between January 2021 and December 2023.* -
  • Both patients, one male and one female, were diagnosed in early infancy and showed symptoms like neonatal cholestasis and hepatomegaly, with specific blood test results indicating direct hyper-bilirubinemia but normal bile acid levels.* -
  • Treatment responses differed: the first patient improved with oral chenodeoxycholic acid, while the second, who had more severe liver failure and a potential

Objective: A multitude of factors affect a hospitalized individual's blood glucose (BG), making BG difficult to predict and manage. Beyond medications well established to alter BG, such as beta-blockers, there are likely many medications with undiscovered effects on BG variability. Identification of these medications and the strength and timing of these relationships has potential to improve glycemic management and patient safety.
