Neoadjuvant Therapy (NT) has become the gold standard for treating locally advanced Breast Cancer (BC). The assessment of pathological response (pR) post-NT plays a crucial role in predicting long-term survival, with Contrast-Enhanced Magnetic Resonance Imaging (MRI) currently recognised as the preferred imaging modality for its evaluation. Invasive lobular carcinoma (ILC) is the second-most common histologic subtype of breast cancer, constituting 5% to 15% of all breast cancers. It is characterized by an infiltrating growth pattern that may decrease detectability on mammography and US. Purpose: Contrast-enhanced digital breast tomosynthesis (CEDBT) highlights breast tumors with neo-angiogenesis. A recently proposed CEDBT system with a dual-layer (DL) flat-panel detector enables simultaneous acquisition of high-energy (HE) and low-energy (LE) projection images with a single exposure, which reduces acquisition time and eliminates motion artifacts. Objectives: Contrast-enhanced mammography (CEM) is an accurate competitor for contrast-enhanced breast magnetic resonance imaging (CE-MRI), but the examination is limited by the lack of 3D information. Digital breast tomosynthesis (DBT) allows better lesion detection and characterization compared with mammography.
