We describe a case of a 35-year-old man presented at the emergency room of our institution with acute onset of dyspnea and dizziness. He was a body builder and had been using Xenadrine EFX for weight loss reduction. The laboratory analyses were normal. A chest radiograph showed an enlarged cardiac silhouette with clear lung fields. Transtoracic two-dimensional color Doppler echocardiography revealed a diffuse hypokinesia with a marked decreased in systolic function and a high teledyastolic diameter. This case document the possible relation to use of Xenadrine EFX for weight loss and the recurrence of dilated cardiomyopathy.

Several supplements and herbal products have been shown to increase catecholamines and subsequently mean arterial pressure and systemic vascular resistance. Since ephedra-containing products have been removed from the market, manufacturers of weight loss herbal supplements must produce ephedra-free formulations. Xenadrine EFX is an ephedra-free weight loss product containing a mixture of caffeine, guarana, and bitter orange (standardized to synephrine).

Following the 2004 ban of ephedra, which was linked to several cases of stroke and myocardial infarction, manufacturers have marketed "ephedra-free" weight loss products that include the active ingredient synephrine. Despite the lack of data on safety and efficacy, synephrine is touted by its promoters as a "safe" alternative to ephedra and is often combined with caffeine, as in the supplement Xenadrine-EFX. We report a case of left middle cerebral artery vasopasm and stroke in a young healthy patient in the setting of Xenadrine-EFX use.
