One hundred Gram-positive bacteremia organisms from five important genus groups were tested against 10 newer beta-lactams. Ceftazidime was significantly less active (50% of strains at less than or equal to 8 micrograms/ml) compared to other cephalosporins. The penems (FCE-22101 and HRE-664) and imipenem were each superior to the cephalosporins with 92-93% inhibition of strains. A novel fused co-drug of fleroxacin and desacetyl-cefotaxime, Ro 23-9424, was 100% effective against these Gram-positive pathogens at less than or equal to 8 micrograms/ml. Several of these compounds should receive consideration for clinical trials for empiric therapy among neutropenic patient infections where Gram-positive pathogens may be more prevalent.

Purpose Of Review: The aim is to discuss the evidence and recent literature on the role of older antibiotics in the treatment of skin and soft tissue infections (SSTIs).

Recent Findings: The choice of therapy for SSTIs is complicated in view of the rising antimicrobial resistance (AMR) and the availability of new antibiotics. SSTIs are predominantly caused by Staphylococcus aureus and beta-hemolytic streptococci, but other organisms can be involved in patients with comorbidities or post trauma.

Carbapenemase-producing carbapenem-resistant Enterobacterales (CP-CRE) represent a significant global threat. The emergence of dual CP-CRE is particularly alarming, as they can potentially compromise the efficacy of newer antibiotics, further decreasing therapeutic alternatives. Herein, we report the emergence of multiple species of CP-CRE recovered from invasive infections in Chile that simultaneously harbor and and provide an in-depth genomic characterization of these worrisome pathogens.

This study investigated a strain of Klebsiella pneumoniae, identified as GRTHES, which exhibited extensive antibiotic resistance. The strain was resistant to all beta-lactams, including combinations with newer agents such as meropenem/vaborbactam and imipenem/relebactam, as well as to aminoglycosides, fluoroquinolones, fosfomycin, trimethoprim-sulfamethoxazole and colistin. It remained susceptible to tigecycline.
