Critical illnesses affect millions of individuals annually in the United States. As advances in patient care continue to improve, the number of survivors is rapidly growing. Critical illness survivors endure profoundly severe illnesses and live through often frightening experiences throughout the course of ICU hospitalization, resulting in a variety of "survivorship" challenges, expressed through a condition known as post-intensive care syndrome (PICS). Questions abound regarding the ideal protocols for ensuring the best physical, cognitive, and psychological outcomes for these survivors. Organizational change is likely to be a key factor, though the specific components have not yet been established. Throughout this article, we highlight some of the barriers and facilitators to enhancing patient care across the spectrum of critical care environments, while also highlighting the challenges inherent to studying a complex patient population. We address each of the areas potentially affected by critical illness and ICU hospitalization--physical, cognitive, and psychological functional domains--experienced by patients as well as their family caregivers.

Background And Objective: To determine whether there is disproportionate reporting of hepatobiliary disorders in the United States (US) FDA Adverse Event Reporting System (FAERS) for individuals prescribed ketamine or esketamine.
Design: We identified Medical Dictionary for Regulatory Activities (MedDRA) terms in the FAERS related to hepatobiliary disorders.
Main Measures: Formulations of ketamine and esketamine were evaluated for the proportionality of reporting for each hepatobiliary disorder parameter using the reporting odds ratio (ROR).

Asking participants to Think Aloud is a common method for studying conscious experience, but it remains unclear whether this approach alters thought qualities-such as meta-awareness, rate of topic shifts, or the content of thoughts in task-absent conditions. To investigate this, we conducted two studies comparing thinking aloud to thinking silently. In Study 1, 111 participants alternated between 15-minute intervals of verbalizing and silently reflecting on their stream of consciousness in a counterbalanced design.

It has become widely accepted that standard connectionist models are unable to show identity-based relational reasoning that requires universal generalization. The purpose of this brief report is to show how one of the simplest forms of such models, feed-forward auto-associative networks, satisfies two of the most well-known challenges: universal generalization of the identity function and the reduplication rule. Given the simplicity of the modeling account provided, along with the clarity of the evidence, these demonstrations invite a shift in this high-profile debate over the nature of cognitive architecture and point to a way to bridge some of the presumed gulf between characteristically symbolic forms of reasoning and connectionist mechanisms.
