CT of appendicitis. Diagnosis and treatment.

CT may provide valuable information in patients with appendicitis whose clinical presentations are atypical. The abnormal appendix and inflammatory changes in the pericecal fat are shown directly rather than inferentially. CT can reliably distinguish phlegmonous inflammation from a liquified abscess and can accurately delineate the full extent of such inflammatory masses. Percutaneous catheter drainage of well-localized appendiceal abscesses under CT guidance is safe and effective and has a lower morbidity than surgical drainage.

Purpose: This study evaluates the effectiveness of machine learning (ML) algorithms for improving the preoperative diagnosis of acute appendicitis in children, focusing on the accurate prediction of the severity of disease.

Methods: An anonymized clinical and operative dataset was retrieved from the medical records of children undergoing emergency appendectomy between 2014 and 2021. We developed an ML pipeline that pre-processed the dataset and developed algorithms to predict 5 appendicitis grades (1 - non-perforated, 2 - localized perforation, 3 - abscess, 4 - generalized peritonitis, and 5 - generalized peritonitis with abscess).

A clinical isolate, R131, was isolated from the peritoneal swab of a patient who suffered from ruptured appendicitis with abscess and gangrene in Hong Kong in 2018. Cells are facultatively anaerobic, non-motile, Gram-positive coccobacilli. Colonies were small, grey, semi-translucent, low convex and alpha-haemolytic.

Stump appendicitis is a known post-appendectomy entity causing right lower quadrant abdominal pain. Usually, a patient with a prior history of appendectomy presents to the emergency room with right lower quadrant abdominal pain and stump appendicitis, which is visualized on computed tomography of the abdomen pelvis. We report a case of stump appendicitis diagnosed by colonoscopy and subsequently confirmed by surgery.
