Published: March 2017

Purpose: The purpose of this study was to conduct a meta-analysis to determine whether the locking plate or non-locking plate results in better opening-wedge high tibial osteotomy (OWHTO) outcomes.

Methods: The MEDLINE, EMBASE, COCHRANE, and KOREAMED register databases were searched for studies. The eligibility criteria for inclusion in the review were studies that compared the locking plate with the non-locking plate for OWHTO, and those that provided clear descriptions of surgical techniques and outcomes. The key outcomes of interest were union rate, correction loss angle, correction loss rate, and full weight-bearing starting point. The statistical software "RevMan" was used in statistical analysis.

Results: Five studies were included in the meta-analysis. Among their reported results, there were no differences in the incidence of union [risk ratio (RR) = 1.01, p = 0.34], non-locking plate was associated with lower incidence of correction maintenance (RR = 1.13, p = 0.0006) and greater angle of correction loss [mean difference (MD) = -2.06, p < 0.00001], and locking plate was associated with a significant improvement in Knee society score and function score (MD = 5.77, p < 0.0001; MD = 7.50, p = 0.0005).

Conclusions: Locking plate provides better clinical outcomes and reduced correction loss rates and angles as compared to non-locking plate for fixation with OWHTO.

Level Of Evidence: IV.

Background: Ulnar shortening osteotomy (USO) is a well-established surgical technique for ulnar impaction syndrome and triangular fibrocartilage complex injuries, but complications like delayed union and nonunion are often encountered. Transverse and oblique osteotomy techniques are commonly used, yet direct comparisons using advanced implants are limited. This study aims to compare the clinical and radiological outcomes of USO using the Jplate with a transverse osteotomy device and the APTUS Wrist Ulna Shortening System 2.

Aim: To determine the optimal first proximal screw position which permits proximal tibial fragment rotation adjustment while minimising osteotomy gap formation when a manual reduction technique is used for TPLO in dogs.

Methods: TPLOs were performed on bone models using Synthes 3.5-mm TPLO implants with a jig but without the use of an anti-rotational pin.

Article Synopsis
  • - The study aimed to compare the effectiveness of locking plate fixation versus conventional plate fixation in treating acetabular fractures, particularly in cases with poor bone quality.
  • - A T-shaped fracture was simulated in synthetic pelvic models, and results showed that locking plates resulted in less displacement of the fracture compared to conventional plates during loading.
  • - Although locking plate fixation reduced displacement, both fixation methods exhibited no significant difference in initial axial stiffness, which is crucial for fracture stability.

Osteosynthesis using Scorpion plate for neer type V distal clavicle fracture.

Department of Orthopaedic Surgery, Ashikaga Red Cross Hospital, 284-1 Yobe-cho, Ashikaga-shi, 326-0843, Tochigi, Japan.

Background: Neer type V distal clavicle fractures are considered the most unstable fracture type and are characterized by the disruption of continuity between the coracoclavicular (C-C) ligaments and proximal or distal bone fragments. However, owing to the rarity of such fractures, there is currently no universally accepted surgical procedure for their treatment. Recently, the scorpion plate, an anatomical, non-locking, pre-contoured plate with two grasping arms to fix the distal or inferior clavicular fragments, was introduced.

Impacts of post-operation loading and fixation implant on the healing process of fractured tibia.

Healing of tibia demonstrates a complex mechanobiological process as it is stimulated by the major factor of strains applied by body weight. The effect of screw heads and bodies as well as their pressure distribution is often overlooked. Hence, effective mechanical conditions of the healing process of tibia can be categorized into the material of the plate and screws, post-operation loadings, and screw type and pressure.
