Objective: To summarize the computed tomography (CT) and magnetic resonance imaging (MRI) characteristics of ovarian fibro the coma and to compare them with the pathological findings.

Method: CT and MRI features of 25 patients with pathologically proved ovarian fibrothecoma were retrospectively analyzed.

Results: Of these 25 patients, the tumors were single in 23 patients and bilateral in 2 patients. The tumors were round or oval in 16 cases and lobulated in 11 cases. CT plate scanning showed that both the solid masses and the solid components of the cystic and solid masses had slightly lower densities than that of the myometrium, and gradual and mild enhancement could be found in the arterial phase and delay phase after enhanced scan. MRI showed iso-low signal on T1-weighted imaging, slightly low or high signal on T2-weighted imaging of fat suppression sequences, slightly high signal on diffusion weighted imaging and the enhance characteristics as the well as CT after enhanced scan.Pelvic cavity effusion was seen in 3 cases.

Conclusion: Ovarian fibrothecoma have certain imaging characteristics, which are helpful to improve the diagnosis and differential diagnosis of this disease and lower the misdiagnosis rate before operation.

Our case involves a 68-year-old postmenopausal patient with a history of total abdominal hysterectomy and right salpingo-oophorectomy performed 25 years ago. The patient presented with chronic pelvic pain for a gynecological examination. Clinically, a large painless pelvic mass was palpable, likely originating from the preserved left ovary.

Article Synopsis
  • The study discusses two rare cases of postmenopausal women experiencing rapid virilization and high testosterone levels, despite negative imaging results for ovarian masses.
  • Both patients underwent hormone profiling and inconclusive imaging, leading to a suspicion of ovarian tumors, prompting surgical intervention through laparoscopic oophorectomy.
  • Histological examination after surgery confirmed Leydig cell tumors in one patient and thecoma in the other, with testosterone levels normalizing and symptoms improving post-surgery, emphasizing the need for thorough evaluation of androgen excess in postmenopausal women.

We present a 39-centimeter thecoma with ascites and elevated Ca-125 values which is compatible with an atypical/incomplete Meigs' syndrome. Giant ovarian masses with elevated Ca-125 values and ascites are an alarming combination, although Gynecologists should be aware that there are also benign entities that mimic advanced stage ovarian cancer.

Article Synopsis
  • - This case report describes a rare instance of metastatic signet ring carcinoma in the ovary of a 46-year-old Black African woman, occurring alongside a benign fibroma, highlighting the complexity of ovarian lesions.
  • - The patient experienced significant abdominal swelling and was found to have a large mass in the right ovary, along with signs of metastasis in the lungs and liver, as indicated by imaging and elevated tumor markers (CA 125 and CA 19-9).
  • - Surgical intervention involved a total abdominal hysterectomy and bilateral salpingo-oophorectomy, revealing a mix of spindle cells and signet ring cells in the ovarian tissues, illustrating the unique histological findings associated with this case.

Article Synopsis
  • The study focuses on mitotically active cellular fibroma (MACF) of the ovary, detailing its clinical, radiological, and pathological characteristics through the analysis of 11 cases between 2015 and 2022.
  • The median patient age was 53.7 years, with most presenting symptoms being abdominal pain; tumor sizes varied, and preoperative imaging often misidentified the tumors.
  • Results indicate that MACF generally has a benign outcome, and fertility-sparing surgical options are effective, with no disease relapses observed during follow-up, highlighting the importance of intraoperative frozen section for accurate diagnosis.
