Residual defect after surgical closure of atrial septal defect is extremely uncommon. This communication reports four cases encountered in a tertiary care center during the last three decades. Background: Post-pneumonectomy bronchopleural fistula (BPF) is a life-threatening event whose treatment is not standardized.
Case Presentation: We report the management of a 28-year-old patient with a 3-year history of BPF complicating right pneumonectomy for congenital emphysema. Despite closure by an Amplatzer device, the patient had chronic pyothorax and severely deteriorated general health and quality of life.

Ventricular pseudoaneurysm (PSA) is a ventricular outpouching contained by adherent pericardium or myocardial scar tissue and represents a rare but potentially fatal complication of acute myocardial infarction (AMI). The vast majority of cases involve the left ventricular apex, in the area of infarct. It is extremely rare to see PSA formation within the interventricular septum (IVS).

We here describe a case which was surgically closed after 10 days but had a failure needing continuous intra-aortic balloon pump (IABP) support and was subsequently closed percutaneously with a Post MI-VSR device.
