Objective: A pilot study was performed to investigate the utility of basic fibroblast growth factor (bFGF) in the repair of blast-induced total or near-total tympanic membrane perforations (TMPs).

Study Design: Prospective clinical study.

Setting: Tertiary university hospital.

Subjects And Methods: Patients who fulfilled the inclusion criteria were treated with 0.10-0.15 mL of bFGF solution applied directly to total or near-total TMPs once daily until the perforations closed or for a maximum of 6 months. The treatment response was monitored via serial otoendoscopy, and audiometric outcomes were evaluated.

Results: Complete TMP closure was achieved in 16 of 17 patients with a blast-induced total or near-total TMP. The mean closure time was 28.4 ± 10.9 days. The improvement in hearing from pre- to post-treatment was statistically significant. There were no complications or adverse outcomes.

Conclusions: The direct application of bFGF to blast-induced total or near-total TMPs is a promising, minimally invasive alternative to conventional tympanoplasty, with a comparable success rate. As reported in the literature, the closure rate was higher than achieved with spontaneous healing. There was no effect of the inverted edge on healing outcome. The use of bFGF in this setting has immediate therapeutic applications for military personnel with blast-induced TMPs who are stationed in isolated, remote environments.

Introduction: Without disease-modifying interventions, Medicare and Medicaid spending on Alzheimer's disease (AD) management is expected to reach 637 billion USD annually by 2050. The recent advent of promising AD therapies after decades of a near-total failure rate in clinical trials suggests that more disease-modifying therapies are on the horizon. In this review, we assess the late-stage pipeline of disease-modifying candidates for AD and offer a novel classification of intervention candidates by treatment paradigms-groups of candidates that share an underlying biological mechanism of action and general disease target.

Article Synopsis
  • The study defines anatomical landmarks using preoperative MRI for patients with pituitary adenomas, focusing on how these landmarks relate to tumor resection rates and potential recurrence.
  • A review of 626 patients treated via endoscopic endonasal approach was conducted, categorizing anatomical landmarks and utilizing statistical analysis to create a predictive model for surgical outcomes.
  • Results showed a high gross total resection rate of 91.05%, identifying key anatomical landmarks and factors significantly associated with tumor progression, supported by a strong predictive model's accuracy.

Department of General Surgery II, Suzhou Hospital, Affiliated Hospital of Medical School, Nanjing University, Suzhou, 215008, Jiangsu, China.

This study aimed to determine whether lobectomy is appropriate for the long-term prognosis of 1-4-cm-sized differential thyroid cancer (DTC). This retrospective study included 2,178 patients with 1-4-cm DTC treated via thyroid lobectomy (LT) or total or near-total thyroidectomy (TT) and who were followed up for at least 3 years. The primary endpoint was a structural incomplete response, which was confirmed via imaging with or without a positive histological or cytological examination.

Purpose: We aimed to present our surgical experience and the impact of a solid or cystic morphology of cerebellar pilocytic astrocytoma (cPA) on surgery and the risk for a re-resection.

Methods: We retrospectively analyzed all children operated at our institution between 2009 and 2023 for cPA. Tumours were categorized into 4 groups: (i) cystic PA without cyst wall enhancement, (ii) cystic PA with cyst wall enhancement, (iii) solid tumour, (iv) and solid tumour with central necrosis.

Objective: The extent of resection (EOR) is an important prognostic factor for both low- and high-grade gliomas. Intraoperative MRI (iMRI) has been used to increase the EOR in glioma surgery. While a recent study reported differences between iMRI and early postoperative MRI (epMRI), their specific relationship to postoperative clinical symptoms remains unclear.
