Alitretinoin in Dermatology-An Update.

Indian J Dermatol

Department of Dermatology, Sri Ramachandra University, Chennai, Tamil Nadu, India.

Published: November 2015

Alitretinoin is a pan retinoic acid agonist. It was initially used as 0.1% gel in the management of localized Kaposi's sarcoma. At present, the use of systemic alitretinoin has proved extremely efficacious in the management of recalcitrant chronic hand eczema. Furthermore, there have been other retinoid responsive dermatosis that have demonstrated remission post usage of systemic alitretinoin. With a better toxicity profile, compared to the other systemic retinoids, alitretinoin could be considered a valuable treatment option in the near future for the treatment of these dermatologic disorders.

Article Synopsis
  • Hand eczema is a common skin problem that can be painful and make it hard for people to work, and there isn’t a clear treatment plan agreed on by doctors in the UK.
  • This study aimed to compare two treatments, alitretinoin and ultraviolet therapy, to see which works better after 12 weeks for patients with severe hand eczema that didn't improve with regular medicine.
  • The results showed that while both treatments helped, alitretinoin was less effective than ultraviolet therapy in reducing the severity of the eczema after 12 weeks.

Hidradenitis suppurativa (HS) is a debilitating chronic skin disorder characterized by painful inflammatory nodules, abscesses and sinus tracts involving intertriginous areas and has an adverse impact on patient quality of life. Over the past decade, the therapeutic options of HS have increased significantly to comprise multiple modalities, including topical medication, systemic therapies (mainly antibiotics, retinoids, and biologics), surgical approaches, and lifestyle modifications. Biologics alone or in combination with surgery remain the treatment of choice for moderate to severe disease.

Article Synopsis
  • Vesicular hand eczema (VHE) has shown limited response to the standard treatment alitretinoin, highlighting the need for new treatment options.
  • The study constructed a gene network from differentially expressed genes and quantitative trait locus genes linked to hand eczema to explore biological mechanisms.
  • By analyzing drug-gene interactions, the researchers identified 123 potential drug repurposing candidates, with a focus on vitamin D analogues, retinoids, and immunomodulating drugs as the most promising treatments for VHE.

Efficacy of alitretinoin in the treatment of Darier disease: a case report.

Acta Dermatovenerol Alp Pannonica Adriat

December 2023

Department of Dermatology, University Medical Center Ljubljana, Ljubljana, Slovenia.

Darier disease is a rare autosomal dominant genodermatosis that initially first presents in adolescence with scaly reddish brown keratotic papules and plaques with a seborrheic and intertriginous distribution. The absence of specific targeted medications complicates the treatment process, and managing resistant cases can prove challenging due to recurrent exacerbations that may result in serious complications such as secondary bacterial and viral infections. Treatments of choice include antiseptics, topical corticosteroids, and systemic retinoids, mainly acitretin and isotretinoin.

Article Synopsis
  • - Prurigo nodularis (PN) is a severe skin condition marked by itchy red or purple lesions, and treating it effectively requires a combination of therapies that tackle both immune and nerve-related issues.
  • - A comprehensive review of 1,153 articles led to the inclusion of 88 studies focused on various systemic treatments for PN, highlighting the efficacy of drugs like dupilumab, thalidomide, and pregabalin among others.
  • - Dupilumab stands out as the most effective treatment with the least side effects, while other medications like thalidomide and pregabalin can cause muscle and nerve pain, and there is a need for more extensive studies on Janus Kinase inhibitors.
