Ingestion of foreign bodies is one of the common causes of emergency department visits. History, physical examination, and noninvasive imaging such as plain radiography and computer tomography scan can guide us regarding the type and location of the ingested foreign body. Most of the patients can be managed conservatively but few may need endoscopic evaluation and treatment.

A highly regio-, enantio- and diastereo-selective strategy involving initial enantioselective conjugate addition to 4-nitro-5-styrylisoxazoles serves as a key step for the desymmetrization of 2,5-cyclohexadienones has been disclosed. We have designed a new class of 2,5-cyclohexadienones appended with 4-nitro-5-styrylisoxazoles to undergo organocatalytic asymmetric double or triple conjugate addition in a domino sequence depending on the substrate type leading to desymmetrization of the 2,5-cyclohexadienone core. The developed protocol allows the construction of a valuable hydrophenanthrene core or a unique bridged scaffold bearing multiple chiral centers with excellent enantio- (up to >99.

When pulmonary arterial hypertension and pregnancy meet: a multidisciplinary clinical experts review.

Arch Gynecol Obstet

December 2024

Department of Clinical, Surgical, Diagnostic and Paediatric Sciences, University of Pavia, Corso Strada Nuova, 65, 27100, Pavia, Italy.

Pulmonary hypertension (PH) is a rare condition characterized by elevated pulmonary arterial pressure and pulmonary vascular resistance, potentially leading to right ventricular failure. Pulmonary arterial hypertension (PAH) is the most common type of PH in women of childbearing age and, as per the modified World Health Organization (mWHO) classification of maternal cardiovascular risk, it falls into mWHO class IV and pregnancy is contraindicated. These patients face an exceptionally high risk of maternal mortality and morbidity, with estimated maternal cardiac event rates ranging from 40 to 100% during pregnancy, because physiological changes happening in pregnancy exacerbate the disorder.

A π-extended cyclobutenofullerene containing an N,N-dimethylanilinoethynyl group was synthesized via a one-pot cascade reaction of C with the corresponding propargylic phosphate. The cyclobutenofullerene was further modified using either one-pot or sequential post-functionalization methods, yielding derivatives containing altered addend structures. During one-pot post-functionalization, hydration reaction of the alkyne moiety continued after the formation of cyclobutenofullerenes.

A three-step synthesis of anti-amoebic, ring-fused mackinazolinones has been developed. A Mannich-type reaction between quinazolin-4-ones and -Cbz propanal in the presence of AgOTf afforded quinazolinones (19-94% isolated yield) bearing a newly formed heterocycle with an alkylamine appendage that, upon -Cbz deprotection and basification, triggered a domino rearrangement to afford 45 separable, ring-fused products. Several compounds inhibited growth of parasites that can cause a lethal human brain infection.

Background Eustachian tube dysfunction is characterized by insufficient dilation, leading to secondary pathologies in the middle ear. By comparing pre- and post-operative grades of Eustachian tube function and nasal resistance measurements, this study seeks to determine if septoplasty can improve Eustachian tube function in cases where nasal septal deviation is likely to cause mechanical dysfunction. We also aim to validate the Jain Bhalerao endoscopic classification of nasal septal deviation by assessing its utility in identifying septal deviations at a higher risk of causing Eustachian tube dysfunction.
